The position of Supported Living Coach exists to ensure that adults with intellectual disabilities live with independence and dignity. The position is a navigator, advocate, barrier breaker, and administrative support for individuals.
The position works with 5 to10 individuals at 20 hours a month, or as assigned. Their focus is based on the individual’s goals from their support plan and their real-life needs, so the job is not just 9 to 5. Also, as well as being cognizant of their individuals’ physical and mental health, and their overall wellbeing. SLCs should always demonstrate respect and advocacy for their clients.
Direct report to the Director of Supported Living
Requirements for the position:
- Bachelor’s degree in a related field or at least 4 years of experience working with people with intellectual/developmental disabilities.
- Must have a valid drivers license, insurance and a car in safe running condition.
- Be able to pass level II and local background check
- Be able to obtain a SLC certificate from Florida.
- Manage housing, utilities, medical care, budgeting and benefits.
- Contacting stakeholders and solving problems with urgency.
- Tailoring supports the client’s needs and not your personal preference.
- Annually: Provide a Functional Assessment for each of your caseloads. Complete an annual packet for your caseload within 10 days of receiving a Service Authorization for a client, and annually thereafter, by the third quarter
- Quarterly prepare all reports needed for each of your caseloads ahead of time and submit after each quarterly meeting.
- Determine how many hours a week your caseload has and develop a schedule to work with each person at least weekly based on their appointments and needs.
- Learn what is a priority and what is important to/for your clients and work on those things first. Keep in mind that any healthcare needs are a priority.
- Each visit to your client’s home, review their medications and initial off on the top of their MAR.
- Assure that the food supply and grocery shopping is going as planned and that everyone is being educated about healthy eating.
- Spend time with your clients at their home educating them on how to cook, clean, do laundry and generally how to maintain their home.
- Assure hurricane supplies have been stored, and procedures have been explained and understood.
Other Requirements:
- Have all weekly documentation submitted by Thursday night each week.
- Assure you complete all assigned hours for clients. If more are needed, bring it to the attention of the Director of Supported Living.
- Text the Director of Supported Living when all the quarterly and support plan meetings have been scheduled.
- Manage your time well and if you need assistance make sure you ask for it.
- Other related duties as assigned.
