Position Description

This position will serve as primary contacts for medically needy patients. Provide appropriate intervention services to
patients and/or clients as well as their families receiving services within the Community Living Assistance and Support
Services Program. Work typically involves a variety of bio-psychosocial issues, assessment of the social needs as
related to the patient's/client's health status, and the development of a plan of services, including advocacy, discharge planning, referrals and practical assistance necessary to accomplish desired objectives. Provides consultation, orientation and training to health care staff on the effects of bio-psycho-social, cultural and economic issues on the management of patient/client health needs. They also serve as a resource referral service in advising clients and their families about available community resources.

Major/Essential Functions

  • Responsibilities include processing provider claims, using computer system to record and report patient information, patient case finding, financial screening, program enrollment facilitation, non-medical counseling, case tracking, and preparing all necessary case documentation.
  • Persons in this position will serve as program spokespeople and representatives to participating health care providers, local communities and local health agencies; therefore, this position requires a positive and professional attitude when working with patients, community leaders, and local health agency and state agency care workers.
  • Persons in this position will be expected to travel between local health-agencies in the fulfillment of their duties and may be asked to make occasional home visits.
  • Work is performed in usual clinic conditions; requires a high degree of contact with patients and families, clinic staff and outside support agencies.
  • Travel and ability to respond to needs during off- duty hours may be required in unusual situations.
  • Performs all other related duties, within the scope of practice, on an as needed basis or as required by Supervisor.
  • Adhere to all TTUHSCEP policies, procedures and processes.
  • Personally demonstrate, display, and act in accordance with TTUHSCEP’s values (Service, Respect, Accountability, Integrity, Advancement, and Teamwork). Actively promote and encourage staff across the institution.
  • Remains current with all licensure, certifications and mandatory compliances and trainings required of this position.
  • Perform all other duties as assigned.

Preferred Qualifications

  • Previous experience working in an academic medical or health sciences center.
  • Bilingual in Spanish/English.
  • Knowledge of social work process, ability to assess client’s needs, ability to compile/maintain information on community resources.
  • Ability to develop positive relationships with clients and collateral agencies.
  • Problem solving and decision making skills.
  • Ability to conduct a client interview and ability to use a personal computer.
