8 a.m. to 5:30 pm Monday- Thursday. 8 a.m. to 12:00 pm on Friday.

8 a.m. to 4:30 p.m., Monday-Thursday (Eudora & Wilmot), Friday 8 a.m-12:00 p.m.

However, additional hours may be required to complete duties.

 

JOB SUMMARY: 

The Care Coordinator is part of the care team and under the supervision of the RN Clinic Manager. The Care Coordinator will coordinate care for patients to providers outside of Mainline Health Systems, Inc. The Care Coordinator is involved in continuous Quality Improvement practices and measures as set by Mainline Health Systems, Inc. and in which the practice focus is based on a Patient Centered Medical Home model of Care.

 

JOB DUTIES:  

Include but not limited to:

  1. Assist patients with referrals to other providers outside of Mainline Health Systems, Inc. and other services.
    1. Make appointments, send referrals to outside providers, complete prior authorization if needed, send Medicaid Referral Form for Medicaid patients, notify patient of appointment, follow-up on appointment, request records, and scan records into EMR. Document all information in the open outgoing referral.
  2. Breast Care
    1. Assist patients in enrollment of Breast Care and entering results into ADH website. 
  3. Interacting with patients and staff on a daily basis.
  4. Communication with management in regards to any related issues.
  5. Notification of patients regarding missed appointments,
  6. Process requests from patients and staff via electronic messages.
  7. Additional duties or responsibilities as deemed necessary.
  8. Participates in monthly clinical staff meetings 
  9. Participate in daily huddles and maintain an active role within the care team
  10. Serve as part of the care team; following and implementing workflows that support and sustain MHSI Patient Centered Medical Home model of care.
  11. Willingness to attend and/or complete continuing education related to this position and/or any other topics as deemed necessary to MHSI.
  12. Completion of continual job training processes set forth by Mainline Health Systems, Inc.

 

QUALIFICATIONS:  

All listed below are key skills necessary for the position.  

  1. High School Diploma or GED is required. Bachelor of Social Work or health care related field is preferred.
  2. Ability to communicate clearly, takes direction, and work well with a team but also self motivated.  
  3. Positive attitude.
  4. Detail oriented.
  5. Ability to navigate and utilize electronic medical records.
  6. Bilingual preferred, Spanish
  7. One year of experience in medical field/setting.

 

WORK ENVIRONMENT:

An office environment in a controlled atmosphere building. This is an OSHA low –risk position.

 

Reporting Relationships:

The Care Coordinator reports to and is directly supervised by the RN Clinic Manager who reports to the Director of Operations.   In the event that the Clinic Manager is not available due to illness or vacation leave, the Care Coordinator reports to the Office Manager or assigned administrative staff.

Evaluation:

The evaluation of work performance will be on going, and will be carried out by the Clinic Manager with consultation of the COO.   The evaluation will include specific duties and responsibilities of the position, plus employee attitudes, general working behavior and willingness to do extra work or assistance.  Formal evaluation will be done annually, but may be initiated at other times by the RN Clinic Manager, Director of Operations, COO, or provider in charge.

 

STATUS:

This is a FLSA Non-Exempt position.

This is an OSHA low risk position.

This is an ADA negotiable position.
