Job Description:
Hospice RN Case Manager – Elara Caring
🕒 Schedule: Monday–Friday | 8:00 AM – 5:00 PM
📍 Position Type: Field-Based | Full-Time

What You’ll Do
Deliver skilled nursing care with empathy and excellence
Develop and update individualized care plans
Monitor health status and adapt care accordingly
Collaborate with physicians, social workers, and chaplains
Support families with education and emotional guidance
Ensure a compassionate, respectful end-of-life experience
What You Bring
Active RN license in the State of Michigan
Minimum of 1 year of nursing experience (hospice or home health preferred)
Clinical confidence, emotional intelligence, and clear communication
Ability to lift up to 50 lbs and travel within your designated region
