Job Description:

Hospice RN Case Manager – Elara Caring

🕒 Schedule: Monday–Friday | 8:00 AM – 5:00 PM
📍 Position Type: Field-Based | Full-Time

As a Hospice RN Case Manager, you’ll provide personalized, compassionate care to patients in the place they feel most at peace: home. What You’ll Do

• Deliver skilled nursing care with empathy and excellence

• Develop and update individualized care plans

• Monitor health status and adapt care accordingly

• Collaborate with physicians, social workers, and chaplains

• Support families with education and emotional guidance

• Ensure a compassionate, respectful end-of-life experience

What You Bring

• Active RN license in the State of Michigan

• Minimum of 1 year of nursing experience (hospice or home health preferred)

• Clinical confidence, emotional intelligence, and clear communication

• Ability to lift up to 50 lbs and travel within your designated region
