ROLE:   Responsible for performing periodic and acute physical, mental, spiritual, and emotional assessments of resident condition and implements appropriate actions.  Supervises staff working in designated neighborhoods. 

 

 

 

 

GENERAL DUTIES & RESPONSIBILITIES:

 

 

 

 

Clinical Care

 

 

 

  • Has knowledge of each resident's diagnosis and current condition in Health Care unit.
  • Responds to emergency calls throughout the Manor campus as necessary.
  • Makes daily rounds on unit's residents.
  • Carries pager and answers call lights and resident alarms promptly.
  • Participates in actual care of residents as necessary.
  • Make assessments and initiates solutions to problems.
  • Assists in the coordination of laboratory work, x-ray work, and physician's appointments as ordered by physician.  May make rounds with physicians.
  • Oversees dining room routine.
  • Assesses and seeks treatment for employees injured at work, ensuring that all proper paperwork has been completed.

 

 

Recordkeeping/Communication

  • Assists in the development of a plan of individualized care for each resident.
  • Assures that each resident's plan of care is implemented, evaluated, and up to date.
  • Ensures direct care staff is informed of residents’ plan of care.
  • Attends resident's care plan meetings as able and documents quarterly progress.
  • Assists in completing designated section of the Resident Assessment Instrument (RAI) assessment for each resident in a timely manner as assigned.
  • Assures that adequate documentation is completed on admissions, dismissals, transfers, medication reviews, monthly summaries, discharge summaries, medication and treatment orders, and physician orders.
  • Notifies physicians, Campus Pastor, Social Work or Life Enhancement Coordinators, and/or family as necessary based on resident's condition/desires.
  • Initiates Resident Change Form and reports status changes and emergencies to Director(s) of Nursing or designee and front office. 
  • Identifies education needs of residents or family and provides necessary teaching and completes the documentation as appropriate.
  • Refers all incident reports and any skin issues/wounds to Unit Managers.
  • Cooperates with ancillary departments, i.e., rehabilitation, therapies, social services, activities, dietary, beauty shop, and maintenance in providing optimum level of care for the resident.
  • Initiates environmental changes that are needed (change of room arrangement, additional cleaning, etc.).

 

Safety

  • Use gait belts and/or mechanical lifts as appropriate and in accordance with policy and procedure.

 

Supervisory/Administrative

 

 

  • Supervises and evaluates the work of CMAs and CNAs in the care of residents and provides knowledge of staff performance as needed to assist others in completing performance evaluations of direct care staff.
  • Evaluates assigned CNAs & CMAs at the 90-day and annual evaluation periods.
  • Attends monthly meetings for lead charge nurses.
  • Creates an atmosphere of team building and working together.
  • Assesses daily assignments and makes appropriate changes.
  • Informs supervisor of staffing problems or concerns.
  • Supervises the documentation of treatments of medications ordered on each resident and assures that needed supplies are available.
  • Responsible for seeing that direct care is provided in a thorough and timely manner.

QUALIFICATIONS:

 

Experience:         Six months of related experience is preferred.

 

 

 

Education:           Registered Nurse in Kansas.

 

 

 

                              Must have current a CPR certification. 

 

 

Skills:                   Enthusiasm, courtesy, and interpersonal skills. 

 

 

Ability to work well with others to achieve desired outcomes.                

 

 

PHYSICAL DEMANDS:

 

 

 

Requires frequent walking, standing, sitting, and bending, as well as frequently lifting up to 30 pounds.
