About the Program
As part of our Health@Home Pathways program, you’ll play a vital role in empowering individuals with complex and chronic conditions to manage their health and wellbeing at home through person-centred, holistic care.
The program is a collaborative initiative across Melbourne’s Eastern Region, delivered in partnership with Eastern Health HARP, Eastern Health Community Health, and Each. By joining this team, you’ll contribute to:
- Reducing avoidable hospital admissions
- Building the capacity of HARP services
- Supporting individuals to confidently manage their health through integrated care coordination
In this role, you’ll directly influence better health outcomes for clients while working alongside dedicated professionals across multiple organisations.
Your role, your impact
As a Registered Nurse Care Coordinator, you’ll be instrumental in supporting patients with chronic conditions to live well at home. You’ll provide comprehensive assessments, develop care plans, and coordinate referrals—ensuring each person receives the care they need to thrive.
Key responsibilities
- Conduct comprehensive health assessments and identify priority issues
- Support patients to set goals and develop care plans
- Initiate referrals to health professionals and services
- Participate in clinical supervision and stakeholder engagement
- Ensure services are high-quality, customer-focused, and evidence-based
- Maintain accurate records and uphold professional standards
- Contribute to continuous improvement and represent the service externally
Who we’re looking for
You’re a compassionate, skilled nurse who thrives in a community health setting. You bring:
- Advanced clinical skills in chronic disease management
- Experience across diverse clinical settings, including general practice
- Commitment to the social model of health and inclusive care
- Strong communication and stakeholder engagement skills
- Qualifications:
- Bachelor of Applied Science (Nursing) or equivalent
- Registered Nurse, Division 1 with current AHPRA registration
- Minimum 4 years’ nursing experience (acute hospital experience preferred).
- Driver’s License
