About the Department

Counsels and assesses the needs of clients and their families who are experiencing complex socio-economic problems or severe family dysfunction. Assists clients in developing a social service plan to achieve their goals and objectives. Coordinates referrals, services, and activities with social service providers

Position Duties

UNDER ADMINISTRATIVE SUPERVISION GENERAL

  • Assesses the needs of clients to determine and develop goals, objectives, and social service plans.

  • Identifies indicators of progress and barriers to achieving family self-sufficiency and family stability.

  • Counsels clients experiencing multiple or complex socio-economic problems, exhibiting severe family dysfunction, or threat of loss of government assistance or housing.

  • Coordinates referrals, services and activities with public schools, community ministries, government agencies, court system, and other social service providers.

  • Monitors and documents the progress of client’s achievement of social service plans.

OSS NEIGHBORHOOD PLACE

  • Assesses the needs of clients and their families.

  • Works with clients to develop service plans that include measurable, achievable, and timely goals.

  • Counsels and educates clients on topics relevant to their goals.

  • Provides referrals and actively works to connect clients with resources in the community.

  • Processes applications for financial assistance.

  • Works with clients to develop discharge plans that include ongoing steps to improve stability.

  • Collects survey data from clients for quality assurance purposes.  

  • Assists Neighborhood Place staff and partners in planning and implementing programs, projects, and events that meet community needs.

  • Attends meetings and events to disseminate information and promote available programs and services.

  • Completes all documentation in an accurate and timely manner.

OSS HOUSING & SUPPORT

  • Assesses the needs of formerly unhoused clients.

  • Assists clients with finding and securing housing.

  • Works with clients to develop service plans that include measurable, achievable, and timely goals.

  • Counsels and educates clients on topics relevant to their goals.

  • Provides referrals and actively works to connect clients with resources in the community.

  • Processes requests for financial and material assistance as needed.

  • Conducts home visits as required by the funding source.

  • Helps clients resolve situations that could lead to eviction.

  • Works with clients to develop discharge plans that include long-term, stable housing.

  • Collects survey data from clients for quality assurance purposes.

  • Assists Housing & Support staff and partners in planning and implementing programs, projects, and events that meet clients’ needs.

  • Completes all required documentation in an accurate and timely manner.

Minimum Qualifications

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:

  • Bachelor’s Degree in Social Work, Psychology, Sociology, or related discipline.

  • Two (2) years of social service experience.

OR 

  • Bachelor’s degree and three (3) years of social service experience

  • Additional education in Social Work, Sociology, Psychology, or a related field may be substituted for experience.


OTHER MINIMUM REQUIREMENTS:

GENERAL

  • Must provide one's own source of transportation


OSS HOUSING & SUPPORT

  • Must provide community-based case management and transportation services to clients as needed. These services include transporting clients to appointments for the purpose of securing housing and accessing community resources. Home visits must be conducted with clients at regular intervals.


PHYSICAL DEMANDS:

Physical demands refer to the requirements for physical exertion and coordination of limb and body movement.

  • Performs sedentary work that involves walking or standing some of the time and involves exerting up to 10 pounds of force on a regular and recurring basis or sustained keyboard operations.
