Pathways to Success is a program for Medicaid-enrolled youth in Illinois who have complex behavioral health needs requiring intensive services. Pathways to Success is an evidenced-informed model of intensive care coordination to support youth through family engagement; individualized care planning; and the utilization of strengths and natural supports. DuPage County Health Department serves as a Care Coordination and Support Organization and operates as a hub for care coordination for Pathways-enrolled individuals within DuPage County.
An individual in the Care Coordinator position will be responsible for providing care coordination and support services to clients enrolled in the Pathways to Success program. The position involves working collaboratively with youth, their families, and community providers to achieve client goals. This full-time position works Monday through Friday, 8:00 am – 4:30 pm, with availability to work at least two evenings a week, 10:30 am – 7:00 pm. The hiring range for this position is $44,400 to $52,000 depending on experience.
Completion of a bachelor’s degree in Psychology, Social Work, or a related field and one year of behavioral health experience; or an equivalent combination of training and experience.
A valid Illinois Driver's License is required as this position will work within the community. Spanish speaking candidate highly preferred.
