On-Call Licensed Vocational Nurse – Hospice

📍 Austin, TX Area | 7 Days On, 7 Days Off
🩺 LVN Required | Hospice Experience Required


CareFor is looking for a skilled, compassionate LVN to support patients when it matters most.

We’re seeking a dependable and responsive On-Call Licensed Vocational Nurse to join our growing team. This role is ideal for someone who thrives in high-touch, high-impact moments and is confident making clinical decisions independently in the field.

This position requires local travel throughout the Austin metro area (Georgetown to San Marcos). The On-Call RN must reside within this region and be available to respond in person during all on-call hours to support Hospice clients.


🕑 Schedule & Compensation:

  • 7 days on / 7 days off schedule for active clinical work during on-call hours

  • On-call hours:

    • Weeknights: 5:00 PM – 8:00 AM

    • Weekends: 24-hour availability

  • Daily compensation during all on-call days

  • Additional hourly compensation for active clinical work performed


🩺 What You'll Do:

  • Respond quickly and compassionately to after-hours needs from CareFor’s hospice clients.

  • Perform in-person assessments to manage new symptoms, urgent needs, or changes in condition.

  • Administer medications and treatments per plan of care or physician orders to relieve pain and manage distressing symptoms.

  • Offer crisis intervention and symptom management in coordination with the broader care team.

  • Communicate clearly and empathetically with families, caregivers, physicians, and interdisciplinary colleagues to ensure seamless transitions and continuity of care.

  • Maintain timely and thorough documentation for every patient interaction.

  • This position does not include phone triage responsibilities. A designated RN is always on call to handle all triage-related duties.

  • Can perform a wide range of clinical tasks including wound care, G-tube feedings, trach care, and more.

  • Take pride in your clinical judgment and your ability to quickly assess what’s needed and deliver it.

  • Are comfortable navigating diverse home environments and medical situations.

  • Understand the importance of confidentiality and strictly adhere to HIPAA and professional ethical standards.

Required Qualifications:

  • Active Licensed Vocational Nurse (LVN) license in the state of Texas

  • Residency within the Austin area (Georgetown to San Marcos)

  • Reliable transportation and readiness to travel locally at a moment’s notice

  • Availability to work 7 days on / 7 days off, including evenings, overnights, and weekends

  • Previous clinical experience in hospice or palliative care

  • Comfort providing care during end-of-life transitions

  • Strong crisis response and in-home care skills
