Company: Caringhand Home Health Solutions, Inc.
Position Type: Per Diem
Work Arrangement: Field-Based – Patient Homes
Compensation: $160-$180 per visit
Caringhand Home Health Solutions, Inc. is committed to providing high-quality, multidisciplinary care that helps patients achieve the highest possible level of independence in their daily activities. We recognize the importance of comprehensive assessments, individualized treatment plans, and collaboration among healthcare professionals, patients, and families.
We are seeking a compassionate and qualified Physical Therapist (PT) to provide physical therapy services to patients in their homes. In this role, you will assess patients’ functional abilities, mobility, muscle strength, gait, range of motion, and rehabilitation potential. You will develop and direct appropriate therapy programs, educate patients and caregivers, and coordinate with physicians and other healthcare professionals to support patient recovery and independence.
The ideal candidate is organized, self-directed, and committed to providing effective, patient-centered care. You should have strong clinical assessment, communication, and problem-solving skills, along with the ability to work independently and coordinate care in a home health setting.
If you are passionate about helping patients improve their mobility, function, and quality of life, we encourage you to join our team.
Note: This is a field-based, per diem position that requires travel to patients’ homes. Although listed as “remote,” this is not a work-from-home or telehealth role.
