Harbor Behavioral Health (HBH) is a community-based behavioral health organization rooted in Lexington, Kentucky and has now expanded into Hopkinsville. We provide structured, team-based services to adults with serious mental health needs, with a focus on building independence, stability, and meaningful community connection.
Our Therapeutic Rehabilitation Program (TRP) delivers daily group services that support recovery, skill development, and functional improvement through a collaborative model that pairs licensed clinical staff with Peer Support Specialists. The Opportunity
As our Hopkinsville LCSW, you'll be the primary on-site licensed clinician for a growing program. In our peer-led model, Peer Support Specialists facilitate the majority of group services, and you provide the clinical backbone: independent clinical oversight, treatment planning, documentation review, and comprehensive intake and assessment. You'll work in close partnership with our Clinical Director and interdisciplinary team to deliver high-quality, person-centered rehabilitation services.

Location: Hopkinsville, KY (primarily on-site)

Schedule
This is primarily an on-site, weekday role in Hopkinsville, with a two-week rotating pattern:
Week one: approximately three on-site weekdays, plus remote clinical coverage over the weekend
Week two: five on-site weekdays
Every other weekend, you'll provide remote clinical supervisor coverage — reviewing and signing clinical documentation and being available for consultation as needed. Some flexibility is required to meet program and client needs.
What You'll Do
Provide independent clinical oversight of TRP services delivered alongside Peer Support Specialists, including timely review and signature of clinical documentation
Develop and maintain individualized, medically necessary treatment plans that support client recovery
Complete comprehensive intake assessments, psychosocial evaluations, diagnostic formulations, and required outcome measures (PHQ-9, GAD-7, WHODAS, LOCUS, and others)
Conduct suicide risk, safety, and functional assessments, and synthesize findings into admission and level-of-care recommendations
Coach and support Peer Support Specialists to strengthen their documentation and clinical observation skills
Respond to psychiatric crises and behavioral escalation with timely assessment, intervention, and coordination of care
Participate in treatment team meetings, reassessments, discharge planning, and continued-stay reviews
Collaborate with families, guardians, hospitals, personal care homes, and referral sources to support continuity of care
Partner with our billing and prior authorization teams so that documentation and diagnoses support clean claims, authorizations, and appeals
Maintain timely, accurate, audit-ready documentation in the electronic medical record
Requirements
Employment is contingent on successful completion of all HBH pre-employment requirements, including background and registry checks, federal exclusion checks, drug screening, health screenings, license and education verification, and reference checks.
Required
- Master's Degree in Social Work (MSW) from a CSWE-accredited program
- Active, unrestricted Kentucky LCSW license, maintained in good standing throughout employment
- Ability to obtain and maintain credentialing and enrollment with Kentucky Medicaid and all contracted managed care organizations
- Strong clinical judgment, assessment, and treatment-planning skills, with the ability to exercise independent clinical judgment within your scope of licensure
- Strong documentation skills, including detailed, timely, audit-ready notes that support medical necessity
- Ability to collaborate effectively within an interdisciplinary team and to provide constructive clinical guidance to Peer Support Specialists
- Valid driver's license, reliable transportation, and current auto insurance (occasional travel between HBH sites and to community partners)
Preferred
- Two or more years of post-licensure clinical experience in behavioral health, community mental health, or psychiatric rehabilitation
- Experience with Kentucky Medicaid, managed care organizations, and medical necessity documentation
- Experience in Therapeutic Rehabilitation Programs (TRP), Psychiatric Rehabilitation Programs (PRP), Intensive Outpatient Programs (IOP), or similar community-based settings
- Experience working with adults with serious mental illness, co-occurring disorders, and complex psychosocial needs
- Working knowledge of trauma-informed care, recovery principles, and evidence-based mental health treatment
