KeystoneCare is seeking a compassionate and adaptable Licensed Practical Nurse (LPN) to join our team in a Floating role, providing coverage across multiple patients and locations during evening and weekend shifts.
This position is ideal for an experienced nurse who thrives in a dynamic environment, values flexibility, and is committed to delivering high-quality, patient-centered care in the home setting.

Primary Job Responsibilities:
- Provide skilled nursing care in accordance with physician orders, patient care plans, and agency policies
- Administer medications and treatments as prescribed
- Perform ongoing patient assessments and monitor for changes in condition
- Ensure patient safety, comfort, and overall well-being throughout the shift
- Communicate effectively with patients, families, and interdisciplinary care teams
- Educate patients and families on care plans, equipment use, and safety measures
- Maintain accurate and timely documentation in compliance with agency standards
Requirements
Job Requirements:
- Graduate of an accredited LPN program
- Current LPN license in Pennsylvania
- Minimum of one year experience in home healthcare or a similar clinical setting
- Familiarity with Feeding Tubes, Assisted breathing Devices (Ventilators) and Hoyer Lift
- CPR certification
- Strong communication and interpersonal skills
- Self-motivated with the ability to work independently
- Reliable transportation and valid Driver's License
