Care Coordinator

At Phoenix Home Care & Hospice, Care Coordinators are the operational heartbeat of home care. You ensure that caregivers are in the right homes, at the right time, delivering care aligned with each client’s plan.

This role requires professionalism, compassion, and strong decision-making skills. You will build ongoing relationships with clients and families while supporting field staff and maintaining compliance standards.

Core Responsibilities

Coordinate daily scheduling operations
Support, supervise, and document caregiver activities
Respond to client and employee concerns
Maintain accurate documentation and reporting
Participate in quality and training initiatives

Qualifications

18+ years old
High school diploma or equivalent
Office experience required
Ability to communicate professionally and effectively
Comfortable in a fast-paced, changing environment
