Description
The Home Health RN is responsible for identifying and coordinating patient and family needs, and for providing palliative, preventative, and rehabilitative care to the patient/family unit. The RN utilizes assessment, intervention, and teaching skills to maximize patient comfort and enhance the quality of life for the patient/family unit. This role follows all home care regulatory and compliance requirements and meets all Joint Commission standards.
Provides ongoing assessments of the physical needs of the patient/family/PCC and initiates appropriate interventions or referrals based on the patient’s plan of care.
-Provides accurate, timely, and appropriate documentation of patient/family services in accordance with HomeCare policies and procedures.
-Serves as the case manager to ensure continuity of care and submits all required documentation promptly to meet regulatory and compliance requirements.
-Evaluates services provided by Home Health Aides every two weeks through on-site supervisory visits.
-Participates in providing 24/7 patient care coverage.
-Follows all State, Federal, and Joint Commission regulations and standards.
-Educates employees on regulatory standards and home health service implementation.
-Reviews and coordinates certification and recertification documentation for skilled patients to ensure completeness and accuracy.
-Assists in orienting new HomeCare staff members.
-Evaluates clinical records and prepares required forms for home health billing to the intermediary.
-Assists in developing quality indicators and implementing the home health quality assurance program.
-Participates in survey site visits for home health.
-Participates in Home Care meetings, assists with departmental planning, and supports adherence to the annual budget.
-Performs other duties as requested by the Home Care Manager.
Requirements
Minimum of one (1) year of current active nursing experience.
-Passing grade on a pre-employment computer literacy evaluation.
-For field-based positions: reliable transportation and ability to perform responsibilities in a timely manner; proof of valid automobile insurance.
Preferred Skills & Experience:
-One (1) year in an acute care setting.
-One (1) year of home health experience with understanding of State, Federal, and Joint Commission Home Health Agency Regulatory Standards.
-Intravenous therapy training and experience.
-Experience caring for terminal patients and others with life-threatening illnesses.
-The ability to type 25 wpm.
