Provide care and assistance to residents in assisted living facility by performing the following duties.  Primarily responsible for providing personal care and assisted services according to the written and verbal direction and supervision of the manager and/or professional nursing staff. 

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.  

  • Provide residents with assistance in assisted living facility.   
  • Document care and assistance given on a daily basis to clients and assist supervisor with service plan and related documentation.   
  • Immediately report and record changes in the resident’s condition and needs.   
  • Clean personal hygiene equipment used in the care of the resident.   
  • Assist with ambulation, lifting, moving and transporting of resident, including pushing a wheelchair.   
  • Perform incontinence care.   
  • Assist residents with toileting.   
  • Assist activity director with scheduled resident activities and entertainment including escorting residents to and from activities and toileting residents during activities. 
  • Interact effectively with residents.   
  • Complete resident status checks every two hours.  
  • Explain procedures and treatments to resident to gain cooperation, understanding, and allay apprehension. 
  • Encourage independence in all resident daily living activities. 
  • Assist with dining service as necessary, supervise dining room during meals to ensure resident safety.   
  • Accompany residents on out-of-building activities.  
  • Lock all doors leading to courtyard. 
  • Provide housekeeping services including laundry, making beds, empty garbage, stocking of supplies. 
  • Responsible for mail delivery on assigned wing. 
  • Take temperature, pulse, blood pressure, and other vital signs to detect deviations from normal and assess condition of resident. 
  • Respond to life saving situations based upon policies, procedures, and protocol.  Will notify work director, nurse (if on duty), and calls for ambulance. 
  • Document history and physical assessment for assigned residents. 
  • Participate in department or unit quality improvement activities. 
  • Communicate with residents and families to gain understanding and compassion. 
  • Knowledge regarding the use of emergency call systems. 
  • Maintain a safe environment, assuring resident safety and security. 
  • Restock supplies in resident apartments. 
  • Provide respect, privacy, and dignity to all residents and does not enter apartments inappropriately. 
  • Greet and assist all visitors in a courteous manner. 
  • Adapt to change in a positive and professional manager. 
  • Perform other duties as assigned. 

Requirements

EDUCATION and/or EXPERIENCE  

High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.  Must be a graduate of an approved nurse assistant training program and a successful completion of an approved nursing assistant competency examination.  

LANGUAGE SKILLS  

Ability to read and comprehend simple instructions, short correspondence, and memos.  Ability to write simple correspondence.  Ability to effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization. 

MATHEMATICAL SKILLS  

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.  Ability to compute rate, ratio, and percent and to draw and interpret bar graphs. 

REASONING ABILITY  

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.  Ability to deal with problems involving several concrete variables in standardized situations. 

CERTIFICATES, LICENSES, REGISTRATIONS  

Must be a Certified Nursing Assistant.  Must have and maintain CPR certification. 

PHYSICAL DEMANDS

While performing the duties of this job, the employee is frequently required to stand; use hands to finger, handle, or feel; reach with hands and arms; and talk or hear.  The employee is occasionally required to walk; sit; climb or balance; and stoop, kneel, crouch, or crawl.  The employee must occasionally lift and/or move more than 35 pounds with assistance.  Must also be able to hear alarms.  Must meet employment physical and have a tuberculin skin test annually.  Must have 12 hours of continuous training annually.
