Essential Job Functions
- Outreach and engage residents to assist in addressing their community-based needs.
- Coordinate with discharge planner and resource team in advance of residents planned transition to housing.
- Identify and engage community employment support such as Vocational Rehabilitation, VA and State/County Workforce.
- Work with local providers and referral sources to determine what community resources are available in their area.
- Engage with alumni for locally based graduates.
- Leads the Resource Team and evaluate residents’ reintegration needs, to include transportation, ongoing medical/mental health treatment, furniture, household goods, food. Assist with physical moves of Residents to new housing.
- Provides transportation for clients as necessary for benefit-related appointments.
- Ensure timely & accurate use of all company systems as required while maintaining complete up-to-date records and any future upgrades as directed.
- Performs other job duties as assigned.
Qualifications Summary
- Bachelor’s degree in social work, counseling or similar field preferred.
- Able to provide case management, education and peer group sessions to clients with substance abuse and/or mental health problems.
- Able to advocate and negotiate in an assertive, but tactful manner, both internally and with community agencies.
- Understanding of issues related to homelessness including common physical & mental health conditions, recovery-oriented, evidence-based interventions, complexities within provided services, and trauma-informed care.
- Collaborates cross-departmentally with Coalition staff to grow & contribute to the success of program objectives, culture, & a positive working environment.
- Ability to build and maintain internal and external relationships that motivate, guide, and/or reinforce the performance of others toward goal accomplishments.
