POSITION SUMMARY:

Provides comprehensive oncology pharmacy care for patients of the Sky Lakes Cancer Treatment Center (CTC), including management of both oral and infusion cancer therapies. This position interprets physician prescriptions and medication orders, facilitates and promotes timely, high-quality pharmaceutical care, and ensures the safe and effective use of all oncology agents. Responsibilities include oversight of infusion medication order review, entry, and IV admixture to USP 797 and 800 standards, as well as coordination of oral chemotherapy dispensing, monitoring, patient education, and adherence follow-up. The pharmacist plans and coordinates clinical activities such as drug information, pharmacokinetics, therapeutic monitoring, and provider consultation; serves as a drug information resource to patients, medical staff, nursing, and ancillary personnel; and precepts pharmacy residents and students. This position is managed by Sky Lakes Clinical Coordinator and reports to the Director of Pharmacy Services.

QUALIFICATIONS:

Required:

Current Oregon Pharmacy license or eligible for reciprocity; Hospital/Oncology pharmacy background and experience; Knowledge of pharmacokinetics, drug utilization review, and drug distribution systems related to pharmacy practice; Excellent verbal and written communication skills and experience discussing therapies with physicians; and ability to analyze drug therapy problems to provide patient outcome.  Available for Advanced Board certification training in Oncology practice.  Pharmacy Preceptor Licensure or eligible.

Preferred:

Oncology pharmacy background and experience including PGY1 or PGY2 Residency; Outpatient Infusion background and experience. 

Lic/Reg/Cert: Current Oregon Pharmacist License and Preceptor License or eligible for reciprocity.

TYPICAL PHYSICAL/MENTAL DEMANDS:

Light to medium physical demand.  Must be able to stand/sit for periods of time at computer terminal.  Must be able to respond quickly to demands for delivery to CTC nursing units of STAT medications and code blue situations.  Must be able to read and interpret physician orders.  Communication with nursing staff and physicians orally, via phone and in writing when needed.  Must be able to prepare I.V. admixtures, as needed, utilizing laminar flow hood or biological safety cabinet. 

ESSENTIAL JOB FUNCTIONS:

  • Review, understand, adhere, prepare and dispense all medications using appropriate techniques and following the Medical Center’s/CTC policies and procedures.
  • Design, recommend, monitor, and evaluate patient-specific pharmacotherapy, including both oral and infusion oncology treatments. Maintain thorough knowledge of pumps and DME used by CTC.
  • Provide concise, applicable, and timely responses to requests for drug information from healthcare providers and patients. Provide medication-use education to patients and caregivers.
  • Directly supervises technicians in proper I.V. admixture, labeling and distribution of medications. Utilize pharmacy support personnel effectively.
  • Participate in the established process for assessing, managing, and reporting medication errors.
  • Assume responsibility for the organization’s ongoing adherence to its medication-use policies.
  • Participates in care planning process including Oral and IV medications:
    • Coordinate oral/IV chemotherapy management, including onboarding, education, adherence monitoring and communication with specialty or external pharmacies
    • Reviews current medications, address drug-drug and drug-disease interactions, adherence, side effects and toxicity; Document follow-up and collaborate with Providers to ensure safe and effective therapy.
  • Follows lab work and make clinical decisions regarding results appropriately.
  • Knowledgeable of reimbursement issues: Medicare coverage, 3rd party contracts, Pre-authorization requirements, and split bills and bundling requirements. Performs monthly audits for charge capture and medication usage per physician.
  • Maintains competency in all position responsibilities and voices ideas/opinions in staff meetings.
