Summary:
Provides direct physical therapy, under supervision of the physical therapist, to patients at PACE or in their residence as needed. Implements plan of care.
Essential Duties and Responsibilities:
1. Assists Physical Therapist in developing and implementing individual treatment goals which reflect patient needs
2. Assists Physical Therapist in developing an apt treatment plan on each patient. Reviews and consults with Physical Therapist frequently for needed changes
3. Following review with Physical Therapist, communicated with other member of medical care team and personnel of other departments in providing for patient’s total needs; This includes recommending other ancillary service referrals
4. Implements treatment plan as outlined by Physical Therapist and reports progress in medical record on each visit
5. Provides support and education for patient and family members regarding patient’s PT needs
6. Notifies, as appropriate, physician, nurse, physical therapist, promptly on any changes in patient’s physical or emotional condition, or any significant occurrences
7. Attends and participates in case conferences and agency in-service programs
8. Provides input in establishing department polices
9. Promote awareness and utilization of PACE services to other professionals, physicians, and the community

- Valid state licensure to work as a Physical Therapy Assistant
- Current CPR certification
- Ability to work independently
- Knowledge of Microsoft Office or equivalent programs
- Excellent customer service skills
- Excellent problem resolution skills
- Detail oriented and good organizational skills

Strength: May be required to lift, transfer, physically assist patients to safely move from place to place in and around the patient’s home. Proper body mechanics are expected to be used to maintain clinician and patient safety throughout treatments.
Stamina: May have to lift clients, and spend much of their time on their feet either standing, walking, climbing stairs, walking on indoor/outdoor surfaces. Strength and stamina are needed to do the job.
Working Environment:
A well-lighted and ventilated working area with temperature control system. The noise level in the work environment is usually quiet. Minimal exposure to chemicals and hazardous waste. Minimal exposure to outside weather conditions. Appropriate ergonomic and personal safety equipment is provided as needed.
