Job Summary:
Assume primary responsibility for home health clinical services provided by AmeriCare. Direct, coordinate and supervise all clinical functions. Maintains compliance with federal and state laws and regulations as well as Community Health Accreditation Program standards. Assure compliance with Avow/AmeriCare Policies and Procedures. Monitor job performance and completion of evaluations and counseling of clinical nursing staff. Ensure each patient’s home health care plan is designed to meet the unique needs of the client and family with services tailored to assist them in achieving optimal level of function.

Job Duties:
Responsibilities/essential functions:
The person in this position must be able to perform the following essential job functions with or without reasonable accommodations.

  • Complies with company policy and all federal, state, and local regulatory bodies. 
  • Comply with the Avow Code of Ethics and Compliance Plan. 
  • Detect non-compliance and follow up as appropriate. 
  • Supervise all clinical activities and personnel who provide direct patient care. 
  • Assist with recruiting, hiring, orienting and ongoing in-service education of all staff. 
  • Remain informed of home health regulations and standards as well as management issues. 
  • Coordinates patient care 
  • Coordinates referrals 
  • Ensures patient needs are continually assessed 
  • Ensure the development, implementation, and updates of the individualized plan of care. 
  • Provide guidance and support for supervisory, field and office staff. 
  • In collaboration with the Director of Nursing, conducts nursing and home health aide performance evaluations. Assists with development of new job standards as needed. 
  • Counsel staff appropriately and in a timely manner concerning areas of deficiency as well as areas of excellence and quality improvement. 
  • Ensure coordination of care by all disciplines. 
  • Assure that a reassessment of a client’s needs is performed by the appropriate health care professional (i.e. when there is a   significant health status change in the client’s condition at the physician’s request or after hospital discharge.) 
  • Participate in Quality Assurance Activities, data-based outcomes and QAPI. 
  • Participate in state, local and national organizations, meetings, seminars, workshops and activities relating to the home health profession and health care services as requested by supervisor. 
  • Work with other agencies and promote good community relations through involvement in community events. 
  • Assure service satisfaction through client visits and other measures, as appropriate. Investigate and resolve agency complaints related to nursing or other department if required. 
  • Ensure care delivery meets the needs of the patients and follows professional practice standards. 
  • Develop, implement, follow, review and revise policies and procedures to guide care delivery. 
  • Monitor compliance with applicable Federal, State and Local laws, Professional Standards and principles, established policies and procedures and the plan of care. 
  • Ensure that the professional standards of nursing practice are maintained by all nurses providing care. 
  • All other duties as assigned.

Qualifications


Education/Experience:
Undergraduate degree required. At least two years of home health experience.

Certificates, Licenses, Registrations:
Valid Florida Driver’s License.
Registered Nurse in good standing with the Florida Board of Nursing
Valid/current CPR certification

Supervisory Responsibilities: Supervision of home health nursing staff. Oversite, during all operational hours of all patient care services and personnel.

Computer Skills:
Working knowledge of typing, data entry and word processing software. Database software and general knowledge of e-mail.

Language Skills:
Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents. Excellent oral and written skills. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to write speeches and articles for publication that conform to prescribed style and format. Ability to effectively present information to top management, public groups, and/or boards of directors.

Mathematical Skills:
Ability to work with mathematical concepts such as probability and statistical inference. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.

Reasoning Ability:
Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.

Work Environment:

While performing the duties of this job, the employee is occasionally exposed to outside weather conditions. Home visits are required. The noise
level in the work environment is usually moderate. Patient care visits and supervisory visits in the field required.
