Horizons Mental Health Center is growing in the resources, services and facilities to assist consumers in our community. The Assertive Community Treatment (ACT) Team provides intensive treatment, rehabilitation, and support services to individuals living with serious mental illness and/or serious behavioral health condition(s) and whose needs have not be effectively met by traditional behavioral health services.

The ACT Team Program Assistant provides clerical and office support to the ACT Team, to assist in providing high quality, patient-centered care to the consumers being served.

This role will work Monday to Friday 8:00 AM to 5:00 PM

ESSENTIAL FUNCTIONS:

  • Responsible for all telephone inquiries relating to the ACT Team and triaging messages between program and team members while maintaining confidentiality
  • Communicates effectively with staff within the ACT Team, across the agency, and with community partners
  • Screen all client contacts for crisis, prioritizes and routes to appropriate staff
  • Schedules and cancels client appointments accurately on a daily basis
  • Participates in daily team huddles by taking notes and providing information relevant to client needs
  • Collects and reports on aggregate measures of numbers and type of client contacts for the team for ongoing review and reporting
  • Tracks and reports client hospitalizations and incarcerations
  • Maintains up-to date records for each ACT Team client and communicates missing information to the team for collection
  • Conducts electronic charting audits to ensure all documents are completed in accordance with ACT Team and agency standards
  • Maintain and update program materials, forms and office supplies, monitor postal mail arriving to program
  • Complete all required documentation, training and competency reviews in a timely and accurate manner as defined by the agency.
  • Other activities as assigned.

MINIMUM KNOWLDEDGE AND SKILLS REQUIRED:

  • Familiarity with community mental health services.

MINIMUM EDUCATION AND EXPERIENCE REQUIRED:"

  • High school diploma or GED equivalent
  • Computer proficiency

PREFERRED EDUCATION AND EXPERIENCE:

  • Two years or more professional experience in mental health services, crisis response, or a related field.
  • Two years or more of experience working with educating and teaching clients about self-care, improving daily living activities.

LICENSURE OR REGISTRATION REQUIRED:

  • Employee must pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens.  
  • Within 60 days of employment, the employee must have completed CPR Training, First Aid Training and other assigned trainings.
  • Valid driver’s license, insurance, and a dependable vehicle
