We provide person-centered, evidence-based services that create pathways to housing stability for individuals experiencing homelessness. Position Summary
The Program Manager, PATH Navigation leads a team of Peer Navigators and PATH Navigators who provide street outreach, engagement, and case management for individuals experiencing homelessness and serious mental illness (SMI) and/or substance use disorder (SUD). This role ensures compliance with PATH contract deliverables, oversees data and outcome reporting, and collaborates with Avivo programs and community partners to deliver recovery-oriented, housing-focused services.
What You’ll Do
- Oversee daily operations of the PATH Navigation program and ensure compliance with state and federal PATH guidelines.
- Supervise and coach a team of Peer Navigators and PATH Navigators, providing reflective supervision and performance feedback.
- Coordinate outreach and case management services, including street outreach, shelter, housing procurement, and prevention.
- Ensure accurate data entry and reporting in Credible EHR, HMIS, and PATH Data Exchange.
- Manage program budget and support fiscal compliance with PATH and DHS guidelines.
- Build and maintain a strong, culturally responsive team aligned with Avivo’s mission and values.
- Promote undoing racism and reducing disparities through training and coaching.
Qualifications
What You’ll Bring
Required Qualifications
- Associate degree in behavioral science, Social Work, Public Health, or related field and 3 years of experience in outreach/navigation/case management for individuals experiencing homelessness or behavioral health.
OR - Bachelor’s degree in related field and 2 years of experience in outreach/navigation/case management.
- Minimum 2 years of supervisory or leadership experience.
- Experience with HMIS and/or electronic health records.
- Commitment to participant wellness, housing-focused modalities, and trauma-informed practices.
Preferred Qualifications
- Master’s degree in human services, Social Work, or Public Health.
- Lived experience of homelessness and/or SUD/Mental Health recovery.
- Experience with PATH, SAMHSA, or similar grant-funded programs.
- Bilingual or multilingual skills (Spanish, Somali, Hmong, or other languages).
Physical Requirements
- Ability to walk up to two miles per day in varied environments and weather conditions.
- Ability to lift up to 25 lbs.
- Must have a valid driver’s license, insurance, and access to a personal vehicle (mileage reimbursement provided).
