Full-Time Home Health RN – (M-F, 8:00 AM–4:30 PM)
Are you a compassionate and skilled RN looking to make a meaningful impact in your community? We're looking for a Full-Time Registered Nurse to join our Home Health team. This position offers regular weekday hours and the opportunity to build lasting relationships with your patients while working collaboratively with a supportive interdisciplinary team.
Schedule: Monday–Friday, 8:00 AM–4:30 PM
Status: Full-Time
Position Summary:
In this role, you'll oversee a primary caseload of Home Health patients, providing skilled, professional nursing care as prescribed by the physician and in compliance with all regulatory standards. You’ll complete comprehensive nursing assessments, develop and implement individualized plans of care, and continually evaluate patient progress to ensure the highest quality outcomes. Collaboration with physicians, team members, patients, and families is key to success in this role.
Primary Responsibilities:
Assessment & Care Planning
Conduct thorough and compassionate assessments of patients
Develop individualized care plans in collaboration with physicians
Identify active and potential needs and adjust plans as appropriate
Participate in case conferences and interdisciplinary care planning
Care Delivery & Evaluation
Provide skilled nursing interventions and procedures
Continuously assess and respond to patient needs
Communicate effectively with patients, families, and the care team
Deliver patient and caregiver education to promote wellness and independence
Utilize agency and community resources to support patient care
Maintain timely and accurate documentation in the medical record
Qualifications:
Graduate of an accredited registered nursing program
Bachelor’s degree in Nursing (preferred)
Minimum 1 year of general nursing experience (Home Health, medical/surgical, or critical care experience preferred)
Current RN license in the Commonwealth of Pennsylvania
Ability to work autonomously in a Home Health setting
Valid PA driver’s license and insured, reliable transportation
Current CPR certification
Satisfactory background check and professional references
