DESCRIPTION

  • The RN provides nursing services in a Prescriber/Registered Nurse medical team environment.  
  • Basic duties include a brief history assessment including taking vitals and medication reconciliation prior to meeting with the physician.  
  • The RN also conducts phone triages, administers injections, and reviews lab results as well as manages the Medication Management and the sample medication program.  
  • RN’s are also available to consult with the MI Adult or DD Clinical Teams, which may include conducting nursing assessments, OBRA assessments, and providing nursing related trainings. 
  • Core Nursing Services:                             
    • Gathers data gathering for Medication Review: Vital Signs, BMI, Pregnancy/lactation, laboratory results, and medication reconciliation for all medications not prescribed at MCMHA. 
    • Coordinates with the Primary Care Physician and other external health care providers as needed. 
    • Gives injections when needed.           
    • Reviews Personal Health Review annually (this would be a longer appointment). 
    • Sees medications only consumers quarterly.  
    • Reviews abnormal labs. 
  • Other nursing services 
    • Provides Nursing Assessments and treatment as needed. 
    • Provides OBRA assessments as needed.     
    • Provides trainings to direct care staff and consumers as needed.            
    •  Completes requisite clinical documentation in accordance with required deadlines and guidelines                         

Performs other related duties as assigned                            

  • Maintains individual credentials and participates in education and training necessary to maintain current competency(s) in skill areas essential for implementing job duties and responsibilities.          
  • Enhances agency staff’s understanding of management services and pharmacy policies and procedures. 
  • Follows oral and written direction completing tasks as assigned by due dates. 
  • Maintains working knowledge of software applications used in performance of daily duties.                    

Corporate Compliance Responsibilities:                                

  • Complies with and maintains competence with all applicable laws, regulations, standards, and rules governed under Medicare, Medicaid, the Michigan Department of Community Health (MDCH), Michigan Department of Human Services (MDHS), Joint Commission, and any applicable funding sources.  
  • Understands, adheres and maintains competence with all MCMHA and CMHPSM standards, policies, procedures as they appear in MCMHA’s, Management of Information, Finance-Contract Management, Recipient Rights, Performance Improvement, Administration, Human Resources, Treatment Foster Care, Environment and Provision of Care Policies.  
  •  Complies with and maintains competence with the Health Insurance Portability and Accountability Act (HIPAA) regulations as they pertain to privacy and confidentiality.    
  • Interacts appropriately with Managers and peers.  Performs required duties with an awareness of impact upon the Authority structure as a whole.     
  • Works as a Team Player within the entire Authority Structure. 
  • Keeps abreast of all pertinent federal, state and MCMHA regulations, laws, and policies as they presently exist and as they change or are modified.

Qualifications


Education, Training and Licensing Credentials: 

  • Registered Nurse with requisite licensure by the State of Michigan and education equivalent to a four-year degree preferred.
  • Experience in diagnosis and treatment of individuals with mental illness and persons with developmental disabilities required.
  • Experience with multi-disciplinary treatment teams required.
  • Computer skills required.
  • Experienced in reviewing and analyzing electronic data preferred. 
  • Ability to work with a wide variety of professional staff and other agencies in an outcome data-based manner is an integral part of the job responsibilities. 

Other Competencies and Abilities: 

  • Ability to work in a complex, detail-oriented, deadline structured busy organization.
  •  The following physical and mental abilities are required:  frequent lifting or carrying up to 15 pounds, must be able to hear, frequent standing/walking going up and down stairs due to community outreach basis, ability to communicate effectively in person and/or phone, the ability to read, analyze and interpret information, ability to write reports, correspondence and procedures, ability to maintain consumer charts/records,  ability to define and solve practical problems, collect data, establish facts and draw valid conclusions, ability to influence others, and the ability to conduct patient assessments and treatment. 
  • Demonstrated proficiency in MS Outlook, MS Office, electronic medical records (Encompass II) and such other applications if/when required.  
  • Ability to be trained and successfully complete other computer training as required. 
  • Valid driver’s license, proof of current automobile insurance and driving record determined by the Authority’s insurance carrier to be eligible for coverage if required as part of positions duties.
