Position Overview

Reporting to the Director of Community Engagement, the Hospice Liaison works collaboratively with all Concord Hospital care management and other members of the medical team to identify potential candidates for hospice services. The Hospice Liaison meets with patients/families to explain hospice philosophy and services, assesses clinical status and individual goals of care to determine appropriateness and readiness for hospice. The Hospice Liaison will determine level of care and will work hospital staff to facilitate a smooth transition from the hospital to home, facility, Hospice House. The Hospice Liaison acts as an advocate, educator, and resource for hospice services to members of the medical team, patients, and families. The Hospice Liaison builds strong relationships with hospital staff to ensure quality care coordination.


Essential Functions

  • Builds and maintains strong relationships with key contacts within Concord Hospital - Concord.
  • Serves as an ambassador of Granite VNA in Concord Hospital – Concord for all Agency programs.
  • Understands and facilitate the Granite VNA referral and intake process. Obtains and transfers essential referral information from Concord Hospital Electronic Medical Record (EMR) to Intake department.
  • Physically present and visible on each Concord Hospital unit throughout the day.
  • Accessible and available for any referral questions related to appropriateness of patients and services provided.
  • Acts as resource and advocate for hospice services.
  • Provides clinical assessment/chart review of patients as requested by hospital staff and does hospice informational visits and/or general inpatient admissions at Concord Hospital.
  • Educates patients and families about hospice philosophy and eligibility requirements, and discusses goals of care.
  • Meets regularly with Palliative Care Team to discuss potential patients and provide hospice perspective.
  • Bi-directional communication between hospital contacts, facility contacts, and members of hospice team.
  • Work collaboratively with the General Inpatient (GIP) Nurse to deliver exceptional, responsive service to hospital partners - anticipating needs, facilitating efficient transitions, and ensuring continuity of care.
  • Completes documentation on a daily basis and in timely manner. 
  • Monitors level of satisfaction of Concord Hospital staff and develops plan with Director of Community Engagement to enhance any relationships as necessary. 
  • Utilizes critical thinking skills and resources to problem solve complex cases and educate referral sources on hospice capabilities.
  • Identifies opportunities for process/quality improvement.
  • Establishes and maintains relationships with vendors to order equipment and coordinate infusion therapies as needed.

Other Job Functions

  • Attends in-services/conferences required for the position to improve job performance.
  • Accepts other assignments deemed appropriate for skills and capability.
  • Travel to assigned work locations is expected.
  • Performs job duties in a safe manner in order to prevent injury to yourself and others.

Expected Hours of Work

The employee will be scheduled to work based on the needs of the Agency. 

Work hours generally are scheduled between Monday – Friday: 8 a.m. – 4:30 p.m.  

Requirements

  • Graduate of an accredited school of nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC)
  • Three to four years of recent experience as an RN in home health or hospice
  • Previous experience in business development or care coordination
  • Hospice experience preferred
