Schedule: Monday-Friday 8am-5pm
Pay Range: $16.50-20.46 per hour, dependent on experience
JOB SUMMARY:
A Community Health Worker at Complete Health will assist promoting healthy lifestyles, primary care, and disease management among patients. The incumbent will facilitate wellness screenings to identify gaps in care, healthcare barriers, and appropriate community-based resources. He/she will foster relationships between patients and their integrated care team through appointment reminders and follow-ups. In coordination with the clinical staff, the Community Health Worker will facilitate care plan development, goal setting, disease management, and other educational interventions. Telephonic visits may be applied to patients with intense chronic morbidities or immediate health needs. Prevention campaigns, such as smoking cessation, healthy eating, and stress management; will be performed periodically to support the general patient population. Other duties may be assigned to suit the demands of patients; according to the respective location of the clinical site.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
· Administer wellness and lifestyle assessments, including Patient Activation Measure (PAM)
· Assess social or environmental barriers that may prevent positive health outcomes
· Foster positive relationships between patient and their integrated care team
· Provide one-on-one patient education and support for the management of behavior health and/or chronic disease
· Deliver health promotion campaigns to prevent chronic disease, substance abuse, and untreated behavioral health concerns
· Develop, review, and evaluate patient care plans and disease management goals
· Identify community resources to reduce barriers to care; such as transportation services
· Participate in quality improvement activities by initiating or contributing to monitoring, measuring, analyzing, improving and/or controlling program goals, objectives and/or services.
· Support current incentive, regulatory, and certification requirements through documentation, participation in initiatives, and other activities as directed.
KNOWLEDGE/SKILLS/ABILITIES:
The Community Health Worker must be able to perform each essential duty satisfactorily. The requirements listed are representative of the knowledge, skill, and/or ability required.
A general knowledge of primary care practice and/or community health. Must have previous experience with working with underserved populations (such as the homeless and/or ethnic minorities). A working knowledge or relationships
with local community resources. Must have personal effectiveness and credibility, leadership and collaborative skills, communication proficiency and flexibility. Must be able to work independently and as a Team Player. Excellent verbal and written communication skills. Able to work independently and in a multidisciplinary team.
