Job Overview & Work Site
What You’ll Do
Clinical (50%)
Deliver high-quality home health physical therapy to a manageable caseload
Focus exclusively on therapy; no nursing, case management, or outside disciplines
Benefit from organized, tightly mapped territories with minimal windshield time
Leadership & Administrative (50%)
Onboard and mentor new therapists
Conduct competency assessments and annual reviews
Lead training on documentation, compliance, and best practices
Monitor EMR quality and support staff development
Serve as a liaison between field staff and leadership, fostering a connected and confident team

License & Experience
What You Need:
- Graduate (current or pending) from an accredited physical therapy program
- Active or pending state physical therapist license
- Home Health experience
