Care Coordinator en Samaritan Bethany

Wage Range: $38.22-$45.63/hr., we honor based on years of relevant experience

Hours: Days 8:00am-4:30pm with every 4th weekend rotation

Job description:

As a Care Coordinator at Samaritan Bethany, you will:

• Manage care of residents based on the resident-directed care plan
• Provide professional communication with residents’ interdisciplinary team
• Comprehensively assess, report changes, and track ongoing resident clinical needs
• Supervises direct care staff by providing direction, completing performance reviews and disciplinary action
• Make change to direct care staff schedule to ensure safe resident care
• Able to work in a fast paced environment and handle changes in daily tasks well regarding the resident care and provider follow-up.
• Critically analyze and make effective change that is best suited to benefit the residents care and staff tasks

Requirements:

• Must have a MN RN License and be in good standing
• CPR Certification
• Prior long-term care experience preferred
