ABOUT NEXUS NEURORECOVERY CENTER – CONROE
Situated on a peaceful, wooded 26-acre campus, Nexus Neurorecovery Center – Conroe offers the Rise Program, a comprehensive neurological rehabilitation program for adults recovering from severe traumatic brain injury (TBI), stroke, spinal cord injury, and other neurological diagnoses. This campus is also home to the Life Care Program, which provides long-term residential care for individuals who have sustained serious, life-altering injuries and require ongoing clinical and behavioral support. POSITION SUMMARY
The Clinical Liaison serves as the primary point of contact between Nexus Neurorecovery Center and referral sources, patients, and families. This position is responsible for identifying, evaluating, and coordinating patient referrals to ensure appropriate clinical placement. The Clinical Liaison works closely with physicians, case managers, hospitals, and other healthcare professionals to promote Nexus programs and facilitate smooth transitions of care.
KEY RESPONSIBILITIES
Referral Development & Relationship Management
Build and maintain strong relationships with hospitals, rehabilitation centers, physicians, and case management teams to generate appropriate patient referrals.
Conduct professional outreach visits and presentations to educate referral sources about Nexus Neurorecovery Center’s programs and admission criteria.
Collaborate with the admissions and marketing teams to support census growth initiatives.
Clinical Assessment & Coordination
Perform clinical pre-admission screenings and evaluations to determine medical and behavioral appropriateness for Nexus programs.
Review patient records, medical history, and psychosocial factors to assess fit for services.
Coordinate with nursing, therapy, and medical staff to ensure smooth and timely admission processes.
Serve as a clinical resource and advocate for patients and families throughout the referral and admission journey.
Documentation & Communication
Accurately document referral activity, assessments, and communication within the admissions database.
Provide regular updates to leadership regarding referral trends, barriers, and outcomes.
Maintain compliance with HIPAA and all organizational privacy standards.
QUALIFICATIONS
Current licensure as an RN, LVN, LCSW, SLP , PT, OT, or equivalent healthcare professional license required.
Minimum of 3 years of clinical experience, preferably in rehabilitation, brain injury, or post-acute care settings.
Prior experience in marketing, admissions, or business development preferred.
Strong knowledge of neurological and rehabilitation diagnoses.
Excellent communication, presentation, and relationship management skills.
Ability to work independently, travel to referral sources, and manage multiple priorities.

Full-time, Monday–Friday
