POSITION SUMMARY:
The Hospice of Baton Rouge is seeking a compassionate and skilled Occupational Therapist to join our dedicated team. This individual will be responsible for meeting and assessing patients in their home setting to implement strategies that identify and prioritize problem areas. This role focuses on helping participants achieve greater safety, independence, and overall quality of life through functional assessments, home modifications, and patient education.
JOB RESPONSIBILITIES:

Conduct structured clinical interviews to identify and prioritize problem areas.

Encourage participants to identify key target areas and engage them in problem-solving to address behavioral and environmental factors affecting performance.

Observe and discuss participants’ functional performance for safety, efficiency, and environmental barriers in each targeted area.

Develop prioritized work orders that include home modifications, repairs, or necessary items to help participants achieve their goals, communicating as needed with handy workers and landlords.

Reinforce strategy use, review problem-solving, refine strategy implementation, and provide education and resources to address future environmental needs during final sessions.

Maintain and manage participant equipment, including inventory, ordering, and transportation, and monitor handy worker activities according to established work orders.

Communicate and coordinate with the CAPABLE nurse to ensure continuity of care.

Assist in resolving participant complaints related to home modifications and equipment.

Consult with physicians and other healthcare professionals to develop and modify participant goals.

Provide detailed therapy interventions that require significant independent judgment in participant care.

Educate participants and families on the proper use of equipment and techniques to promote independence.

Maintain accurate and thorough records, reports, and forms in accordance with documentation policies.

Actively pursue continuing education in areas of occupational therapy to maintain professional knowledge and licensure.

Maintain current occupational therapy licensure and required continuing education units (CEUs).
Qualifications
Education:
- Bachelor's or Master's Degree in Occupational Therapy from an accredited program.
Certification:
- Current Louisiana License from the Board of Occupational Therapist
- Current CPR certification
Work Experience:
- At least two years of experience as an occupational therapist in a clinical setting.
Special Skills:
- Ability to assess and respond to the needs of participants.
- Ability to work independently as well as on an interdisciplinary team.
- Supportive and sensitive to the needs of participants and families, as well as other staff.
- Able to cope with the emotional stress of participants, family, and self.
- Humanistic attitude toward participants /family care. Tolerant of individual lifestyles.
Other Requirements:
- Good physical assessment skills and computer skills.
- Good communication skills.
- Installation of company communication application on cellular device.
- Must have own transportation and provide proof of liability insurance which meet state requirements.
- Must be able to travel to and from participant's homes without assistance.
