PURPOSE OF THIS POSITION

The purpose of this position is to supervise the day to day clinical and administrative operations of the inpatient Palliative Care team, in coordination with the Palliative Care Manager, Palliative Care Physicians and APPs, Palliative Care office team members, Hospice Medical Director, and Hospice leadership and IDG. This position reports to the Palliative Care Manager.

JOB DUTIES/RESPONSIBILITIES

Duty 1: Demonstrates the ability to support the mission of the Palliative Care and Hospice teams in providing consistent, high quality patient care to those being referred to or receiving services.

Duty 2: Demonstrates the knowledge and skill to thoroughly complete both Palliative Care nursing patient encounters and Hospice admissions, following all regulatory documentation and billing requirements for both programs.

Duty 3: Supervises the day-to-day operations and schedules of the Inpatient Palliative Care team across all locations, and coordinates schedules as needed with the Palliative Care outpatient and office staff, Hospice Intake staff, and Palliative Care and Hospice management.

Duty 4: Works collaboratively with both the Hospice management team and Palliative Care management to ensure consistency in program processes and policies.

Duty 5: Collaborates with Palliative Care and Hospice management, as well as the Community Nurse Liaisons in identification and education of agency services to referral sources.

Duty 6: Displays Service Excellence as evidenced by practicing the mission, vision, and values of the organization to promote patient satisfaction and contract agency satisfaction.

Duty 7: Complies with organizational policies, procedures, and practices to ensure quality job performance and compliance with applicable regulatory and accreditation standards.

Duty 8: Completes other duties as assigned.

REQUIRED QUALIFICATIONS

  • Current state licensure as a Registered Nurse (RN)

  • Ability to perform duties of a direct patient care RN.

  • A valid driver's license is required (if you do not have a valid Ohio driver’s license you must obtain one within 30 days of your residency in the state).  You must also meet BVHS's company fleet policy and insurance company requirements, and any other requirements that may be required to operate a vehicle.

  • Positive service-oriented interpersonal and communication skills required.

  • Individual must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served on his/her assigned unit/department. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient status.  Must be able to interpret the appropriate information needed to identify each patient’s requirements relative to their age-specific needs and to provide the care needed as described in the area’s policies and procedures

     

     

PREFERRED QUALIFICATIONS

  • 2 years of Palliative Care and/or Hospice experience in an inpatient and/or outpatient setting with a working knowledge of Home Health Care, Hospice Care, and Palliative Care

  • BSN

  • Prior supervisory experience

 

PHYSICAL DEMANDS

This position requires a full range of body motions with intermittent walking, lifting, bending, squatting, kneeling, twisting, and standing. The associate will be required to walk for up to one hour a day, sit for four to six hours a day and stand for one hour a day. The individual must be able to lift twenty to fifty pounds and reach work above the shoulders. The individual must have good eye-hand coordination and fine finger dexterity for simple grasping tasks. The individual must have excellent verbal communication skills to perform daily tasks. The associate must have corrected vision and hearing in the normal range. The individual must be able to operate a motor vehicle for business travel and community involvement.
