Home Health Registered Nurse – Empath Home Health

Empath Home Health, a member of Empath Health, is seeking a compassionate Home Health RN to provide skilled nursing care for patients in their homes throughout South Hillsborough county & North Manatee county. In this full-time role, you’ll deliver individualized care, build trusted patient relationships, and help people manage their health with dignity and independence.

Schedule: 40 hours; Monday–Friday, with required weekend/on-call/holiday rotations.

Locations: home health visits in South Hillsborough county (Riverview, Gibsonton, Apollo Beach, Ruskin, Sun City Center, Wimauma, Fish Hawk) and North Manatee county (Palmetto, Ellenton, Parrish, Terra Ceia)

What You’ll Do

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled nursing services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

Position Requirements

• Graduate of an approved nursing program with an active Florida RN license
• BSN or MSN preferred
• Minimum of 1 year of direct patient care experience.
• Home health RN experience strongly preferred.
• HCHB (Homecare Homebase) EMR experience strongly preferred.
• Strong assessment, critical thinking, and patient education skills
• Willingness to participate in weekend and holiday on-call rotations
