Hospice Master's Level Medical Social Worker (MSW) — Full Time

We are seeking a Full-Time Hospice Medical Social Worker (MSW) to support patients and families throughout Lebanon County, PA. This role offers the opportunity to build meaningful relationships, advocate for patient dignity, and support the emotional, psychosocial, and resource needs of individuals during one of life’s most significant journeys.

Must hold a Master's Degree in Social Work and an active PA MSW license.

What You’ll Do

As a Hospice Medical Social Worker, you will support patients and families through emotional, psychosocial, and practical needs, including:

• Conduct initial psychosocial assessments and contribute to the plan of care
• Provide ongoing counseling, support, and crisis intervention
• Assist patients and families in navigating grief, loss, and adjustment to illness
• Advocate for patient and family needs and preferences
• Coordinate community resources, referrals, and support services
• Participate in interdisciplinary team meetings (IDG) to ensure holistic care
• Support bereavement services and community outreach when needed
• Maintain timely and accurate clinical documentation (within 48 hours)

• Attend staff meetings, trainings, and IDG conferences as scheduled
• Participate in orientation and continuing professional development
• Uphold hospice philosophy and professional social work standards
• Demonstrate compassion, professionalism, and strong listening skills
