QUALIFICATIONS

Required: Knowledge of pharmacokinetics, drug utilization review, drug distribution systems; and Communication skills and patient assessment skills to provide optimum and safe drug services to patients, families, nursing personnel and physicians. Must have high school diploma or equivalent. Must have high school diploma or equivalent. 

Preferred: Hospital pharmacy background and experience; and outpatient Infusion and anticoagulation management experience. Advanced certification training in anticoagulation. 

Lic/Reg/Cert: Current Oregon Pharmacist registration 

TYPICAL PHYSICAL/MENTAL DEMANDS

Light to medium physical demand. Must be able to stand/sit for periods of time at computer terminal. Must be able to respond quickly to demands for delivery to SLIC nursing units of STAT medications and SLIC/SLAC code blue situations. Must be able to read and interpret physician orders. Communication with nursing staff and physicians orally, via phone and in writing when needed. Must be able to prepare I.V. admixtures, as needed, utilizing laminar flow hood or biological safety cabinet.

ESSENTIAL JOB FUNCTIONS

• Assesses IV medication orders for drug stability, compatibility, patient compliance.

• Participates in care planning process including:

o Identifying problems, determining the appropriate interventions, following outcomes

 o Reviews current medications and is able to address drug-drug and drug-disease interactions

 o Appropriately documents interventions

 

• Clinical expertise in the following areas: Parenteral and Enteral nutrition, Antibiotics/antivirals, Pain Control, Geriatrics, Pediatrics, Oncology and Anticoagulation.
• Follows lab work and makes clinical decisions regarding results appropriately.
• Completes a limited physical assessment and takes a complete patient medication history, if applicable.
• Reviews, understands, and adheres to CTC and SLIC/SLAC policies and procedures and maintains all order sets.
• Provide appropriate patient medication education and follows good bedside manner when interviewing or educating patients.
• Thorough knowledge of pumps and DME used by SLIC and CTC.
• Knowledgeable of reimbursement issues: Medicare coverage, 3rd party contracts, Pre-authorization requirements, and split bills and bundling requirements. Performs monthly audits for charge capture and medication usage per physician.
• Directly supervises technicians in proper I.V. admixture, labeling and distribution of medications.
• Works on project(s) that contribute to the development and subsequent improvement of the Pharmacy Department.
• Maintains competency in all position responsibilities and voices ideas/opinions in staff meetings.
