Responsible for delivery and documentation of professional occupational therapy services to agency patients in
accordance with an established physician plan of care and in compliance with applicable licensure/certification
requirements, professional occupational therapy acts, and agency policies and procedures.
Minimum Education
Graduate of an accredited school of Occupational Therapy.
Minimum Work Experience
One year of clinical experience in occupational therapy in an acute care or rehabilitation setting with
exposure to geriatric and medical/surgical patients.
Required Licenses/Certifications
Current licensure in occupational therapy as required by the state of Texas. Valid Texas driver's license with
available and dependable transportation and proof of current automobile liability insurance. CPR certified.
Evidence of current registration by the American Occupational Therapy Association.
Required Skills, Knowledge, and Abilities
Effective verbal and written communication skills. Ability to meet minimum hiring standards.

JOB SPECIFIC FUNCTIONS
1. Demonstrates an understanding of and adherence to the HMHD Compliance Plan.
2. Conduct reflects HMHD's values and a commitment to HMHD's Code of Conduct.
3. Attends the required corporate integrity and compliance training and education programs.
4. Demonstrates proficiency in understanding the materials presented during the corporate integrity and compliance
training and education program.
5. Complies with all HIPAA standards.
6. Evaluates the patient within 72 hours.
7. Performs initial patient evaluation and plans appropriate therapy program with approval of the attending physician.
8. Initiates the plan of care and makes necessary changes as patient condition dictates and in accordance with
physician's approval.
9. Plans appropriate STG's, LTG's and documents progress in attainment of same.
10. Treats patients for the purpose of relieving pain, developing/restoring functional capability, and maintaining
maximum performance.
11. Directs and aides patients in active and passive exercise, muscle re-education, and functional training activities of
daily living, such as self-care and ADL's.
12. Develops and maintains positive rapport with rehabilitation and hospital staff and physicians.
13. Maintains good working rapport with patient family members.
14. Seeks appropriate medical advice as necessary.
15. Maintains and submits all required patient documentation in a timely manner:
16. Verifies physician's orders prior to initiating treatment.
17. Completes initial evaluation.
18. Completes daily progress notes
19. Documents teaching and patient/family response
20. Completes discharge summaries.
21. Participates in discharge planning.
22. Confers with physicians and staff as necessary; documents all pertinent information and recommendations.
23. Orients, instructs and directs work activities of Assistants and Aides.
24. Develops and instructs Assistants in the established plan of care and any ongoing changes.
25. Provides complete instruction for Assistants and/or Aides performing Occupational Therapy procedures.
26. Supervises care rendered by Assistants and Aides.
27. Plans and conducts lectures and training programs on Occupational Therapy and related topics for Rehabilitation
Services staff, Medical staff, clinical departments, students and community groups.
28. Attends ongoing continuing education programs and in-services.
29. Conducts staff in-services as requested.
30. Attends staff meetings as requested.
31. Fosters professional relationships as evidenced by educational programs and training.
32. Participates in performance improvement activities.
33. A willingness to float in different departments of therapy services for Occupational Therapy, Physical Therapy, and
Speech Therapy services
