We are seeking a professional and dedicated Registered Nurse (RN) to join our team and deliver skilled nursing care while overseeing patient care plans and supervising support staff.
Position Summary
The Registered Nurse (RN) provides skilled nursing care to patients in their homes in accordance with physician orders and the established plan of care. The RN assesses patient and family needs, promotes safety and disease management, maintains compliance with agency and regulatory standards, and supervises Licensed Practical Nurses, Home Health Aides, and Companion Homemakers assigned to the patient’s care.
Qualifications
✅ Graduate of an accredited school of nursing
✅ Licensed and in good standing as an RN in Florida
✅ Minimum of 1 year nursing experience; home care experience preferred
✅ Successful completion of Level II background screening,OIG, and Sex Offender Registry checks
✅ Physical ability to lift, push, or pull up to 50 lbs
✅ Strong communication skills (verbal and written) in English
✅ Ability to learn and apply professional training, follow physician orders, and maintain accurate clinical documentation
✅ Knowledge of infection control, exposure precautions, and disease management
Key Responsibilities
- Provide skilled nursing services, treatments, and diagnostic procedures requiring specialized skills in the home setting.
- Assess and document patient condition, reactions to treatments, and changes in physical or emotional status; report findings to the Clinical Manager or physician.
- Develop, update, and maintain the plan of care, including supervising home health aides and other support staff.
- Educate and train patients and families on disease management and self-care per the plan of care.
- Reconcile and monitor medications each visit; report abnormal findings within 24 hours.
- Maintain confidentiality and comply with HIPAA regulations.
- Submit accurate nursing documentation within 24 hours of shift completion.
- Participate in in-service training, continuing education, workshops, and self-study programs annually.
- Perform wound care, IV care, and other high-tech skills (on-the-job training provided as needed).
- Communicate effectively with patients, families, caregivers, and the Clinical Manager.
- Travel to patient homes in various conditions, maintaining safety and professionalism.
Physical & Mental Requirements
- Ability to lift or assist patients weighing up to 50 lbs and stand/move about for extended periods.
- Able to walk, bend, stoop, twist, and climb stairs as needed.
- Must be able to hear, see, and communicate clearly.
- Able to handle mental and emotional stress of patient care in diverse home environments.
