Position Duties
Primary Job Duties Include:
Resident Care
- Provide direct resident care by hands-on performance or assistance as needed with activities of daily living.
- Provide environmental needs by arranging and adapting personal care items such as food or clothing and/or physical environment needs, e.g. wheelchair, walker, etc.
- Supervising and instruction of residents as needed for self care activities.
- Recommends observation of specific residents who may need dietary changes, e.g. from regular diet to pureed or regular liquid to thickened liquid.
- Monitor safety needs by supervising residents' whereabouts, interactions, and conditions and refer to the charge nurse if necessary
- Collaborate with the charge nurse and other nursing personnel to provide and exchange information regarding resident care status, and evaluate the effectiveness and appropriateness of the Treatment Care Plan, inform the charge nurse of refusals of hygiene care.
- Document observations and care provided on various residents' records.
- Provide psychosocial needs of residents and their significant others by direct personal contact.
- This position will be required to deliver blood draws to the lab, drive a resident to a doctor’s appointment, or to the hospital (if not transported by ambulance).
- Maintain education by attending mandatory and non-mandatory in-services and workshops to keep current as mandated by state law and VA regulations and facility policy and procedures.
Minimum Qualifications
Must have completed the training program for a CNA and must obtain CNA license within four months of hire.
- Required to obtain CPR certification and Vaccinations as required by State and facility.
- Knowledge and ability to appropriately carry out assigned resident care based on the residents' conditions; to use judgment and to accurately recognize, report and record relevant resident information.
- Knowledge and ability to recognize urgent or emergent resident care situations, seek assistance of the RN and/or MD/DON, and initiate appropriate emergency interventions as directed.
- Knowledge of quality standards, procedures, and protocols
- Knowledge of specialized client care applicable to the work situation
- Skill in initiating, performing and completing assigned duties in providing care to residents.
- Ability to effectively work with an elderly patient population of varying physical, mental and emotional abilities and staff from multiple work units in the facility to achieve quality care planning and delivery
- Ability to observe, identify, and respond to the resident's needs for care, equipment-assisted care, and patient/family education.
- Superior verbal, listening, and written communication skills with all healthcare providers, residents, families, and the general public.
- Demonstrated ability to display professional conduct at all times when dealing with fellow staff members, patients, families, and the general public.
- Demonstrated ability to maintain confidentiality at all times.
- Long-term care/nursing home experience is highly desirable
- Effective problem-solving skills

- Full Background: CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference Checks, Professional License verification (licensure requirements), Drug Screen, CAPS (direct contact with adults – Mental Health Institutes, Regional Centers, Veterans Community Living Centers)
- Must pass a TB test for employment
This position requires State of Colorado residency at the time of application (unless otherwise identified in the posting), and residency within the state throughout the duration of employment in this position.
Motor Vehicle Check , if the employee will drive a state-owned vehicle, either in an on-going capacity while performing their normal day-to-day job duties OR on occasion for training/meetings/client visits/etc.
Must possess a valid, non-restricted Colorado Driver’s License or an non-restricted U.S. Driver's License for positions within 30 miles of the CO state border
Out-of-State Driver’s License holders must obtain a valid, non-restricted Colorado Driver’s License within 30 days of employment start date.
