As the HCV Patient Navigation Coordinator, you will play a pivotal role in individuals' lives, making a significant impact through:
- Review completion of daily temperature checks
- Review completion of internal controls according to state and agency guidelines
- Provide Pre and Post-test counseling
- Perform HCV screening and testing in accordance with state, agency guidelines, and contract guidelines
- Perform and support Project Safe Points Point of Care testing through the GeneXpert machines
HCV Patient Navigation Coordinator Minimum Salary: $ 58,500
HCV Patient Navigation Coordinator Maximum Salary: $ 58,500
HCV Patient Navigation Coordinator Schedule: Monday - Friday 8:30 AM - 4:30 PM (37.5 hours per week)
Qualifications:
- Bachelor's Degree in Health Education or Human Services
- 1-3 years of relevant experience in Harm Reduction and related fields
- Valid NYS Driver's License, including a reliable vehicle
