Patient Care

  • Provides basic nursing care to non-acutely ill patients within scope of practice.
  • Addresses both psychosocial and physical needs of patients.
  • Communicates effectively to meet patient needs.
  • Demonstrates initiative and responsibility in patient care.
  • Assists with patient admission, transfer, and discharge.
  • Delivers age-appropriate care, considering patient condition.
  • Alerts licensed personnel when patients report pain.
  • Reports changes in patient status to RN.
  • Supports feeding and hydration based on patient age and condition.
  • Explains procedures to patients and families; answers questions within scope.
  • Performs treatments and services within scope of practice.
  • Collaborates with other departments for interdisciplinary care.
  • Prepares medical records for admissions and discharges.
  • Coordinates nursing staff for patient/physician support.
  • Transcribes and enters physician orders promptly.
  • Communicates clearly with charge nurse, coworkers, and physicians.
  • Manages supply orders and weekly printing needs.
  • Answers phone/intercom courteously; relays messages appropriately.
  • Ensures patient safety and minimizes healthcare errors.
  • Assists physicians with procedures under RN supervision.
  • Adapts to stressful situations with flexibility and organization.
  • Responds to patient emergencies and distress.
  • Treats patients and families with dignity and respect.
  • Involves patients/families in care planning.
  • Maintains positive relationships within and across departments.
  • Completes documentation accurately and within shift.
  • Operates equipment safely and correctly.
  • Supervises patient care under RN direction.
  • Demonstrates understanding of disease processes.
  • Provides care tailored to adolescent, geriatric, and general populations.
  • Practices medical and surgical asepsis during procedures.

 

Safety Practices

  • Promotes a culture of patient safety which results in the identification and reduction of unsafe practices.
  • Utilizes Culture of Safety survey responses in improvement efforts.
  • Identifies risks and hazards to patients through various risk assessments, with actions taken to mitigate patient harm.
  • Improves medication safety practices, including increased reporting of adverse drug events and improved compliance with the medication reconciliation process.
  • Decreased, hospital-acquired infections and improved compliance with infection control practices (including hand hygiene),
  • Decreased hospital acquired conditions such as complications associated with ventilator use and pressure ulcers, through improved compliance with patient safety bundles and/or other formal performance improvement programs.

Professional Responsibility

  • Adheres to dress code; appearance is neat and clean.
  • Completes annual education requirements.
  • Maintains regulatory requirements.
  • Maintains patient confidentiality at all times.
  • Reports to work on time and as scheduled, completes work within the designated time.
  • Wears identification while on duty, uses a computerized punch time system correctly.
  • Completes in-services and returns in a timely fashion.
  • Attends annual review and department in-services, as scheduled.
  • Attends at least 75% of staff meetings annually, reads and returns to all monthly staff meeting minutes.
  • Represents the organization in a positive and professional manner.
  • Actively participates in performance improvement activities.
  • Complies with all organizational policies regarding ethical business practices.
  • Communicates the mission, ethics, and goals of the facility.

 

Education:

  • High School diploma or the equivalent
  • Successful completion of certified nursing assistant program

Experience:

  • Minimum 6 months to 1 year of related work experience preferred.
