Registered Nurse (PRN)
Position Summary
New Start II is a long-term structured residence that serves males with a forensic and mental health history and substance abuse disorders. Our goal is to reintegrate these residents back into the community. We hope to accomplish this goal by working on activities of daily living, such as cooking, cleaning, and budgeting. We also link the residents to service providers who can also help them build relationships in the community. We strive to provide holistic care through group and individual therapy, medication management, psychiatric rehabilitation activities, and evidence-based practices.
Essential Duties and Functions
- Complete daily mental status exams on up to 18 residents.
 - Complete narcotic counts at beginning and end of shift.
 - Administer medication via medication administration record.
 - Identify early signs of psychiatric relapse, medication side effects, or medical issues.
 - Maintain detailed nursing notes in EMR/charting.
 - Document progress toward treatment plan goals.
 - Ensure compliance with state regulations, the Joint Commission, and organizational policy.
 - Lead response to psychiatric or medical emergencies.
 - Make patient appointments and assist in transportation when needed.
 
Clinical
Collaborate on Personal Recovery Plans which emphasize residents’ strengths and needs.
Engage with residents regularly to note changes that may impact the recovery plan.
Educational
Participate in program milieu.
Attend and/or facilitate educational groups and meetings.
Provide education to support residents in understanding medication management.
Provide education to support residents in understanding daily health and wellness needs.
Medication and Health Management
Complete assessments/physicals within regulatory time frames.
Ensure residents’ prescriptions are current and collaborate with MD as needed.
Coordinate medications and health needs with the program Psychiatrist.
Maintain electronic medication administration record (eMAR).
Monitor the administration of all medications.
Assist residents with scheduling medical appointments and follow up on recommendations.
Coordinate physical healthcare with Primary Care Physicians and specialists.
Program
Document educational groups and other resident contact.
Participate in case consultations, staff meetings, and supervision as assigned.
Attend training as required by RHD and licensing bodies.
Perform duties as assigned by director to ensure continuity of care.
Job Qualifications
• Current PA RN/LPN licensure • Current CPR/First Aid certification • Experience with Behavioral Health/Criminal Justice preferred • Valid Driver’s License • Act 33/34 clearances • FBI clearance as needed • All trainings required by licensure  Must complete a Physical, TB test, and Hep B titer.
