Care Team Assistant - Job Overview

Schedule: Monday-Friday (8AM-5PM)

Key Responsibilities

  • Engage in meaningful patient communication, addressing calls with care and ensuring timely follow-up.
  • Support pre-visit planning to ensure critical patient information is always available.
  • Facilitate seamless care coordination by managing care transitions and supporting access to services.
  • Assist with medication management, including refills and prior authorizations, to support patient needs.
  • Collaborate with referral coordination staff to ensure efficient documentation and processes.
  • Champion a culture of respect, teamwork, and safety in all interactions.
  • Adhere to Legacy Community Health's protocols and training to continuously improve safety and operational standards.

Minimum Qualifications

  • High-school level education or equivalent; Medical Assistant Certification is highly valued.
  • At least 2 years of experience in a dynamic ambulatory clinical setting is preferred.
  • Fluency in Spanish is a significant advantage, depending on service location.
  • Proficiency with electronic health record systems (EHRS) is essential.
  • Strong communication and customer service skills to make a positive impact.
  • Ability to excel in a fast-paced, multi-tasking environment with exceptional organizational skills.
  • Proficient typing and computer literacy to support data-driven decision-making.
  • Commitment to managing confidential and sensitive information with discretion.
