Salary Range:    $22.15 - $26.00 Hourly

Description

Zumbro Valley Health Center is seeking a Rehab Worker to join our IRTS program. The rehab worker provides client specific support services to IRTS residents in the areas of mental health, co-occurring disorders, monitoring medication compliance and independent living skills, to monitor behavior to assure client safety and well being, to facilitate residential program activities, classes and groups, and to maintain clean and safe standards for the Intensive Residential Treatment Services and Crisis Stabilization.

Responsibilities

  • Maintain knowledge of community resources. Maintain knowledge of different housing programs offered by Zumbro Valley MHC and understanding of the requirements for each.
  • Timely and accurate completion of: 1) admission and discharge paperwork; 2) financial forms for crisis stabilization residents 3) client treatment plans 4) shift notes 5) individual rehabilitation service notes, and 6) all other forms and documentation required by ZVRT.
  • Complete Crisis Stabilization paperwork within 24 hours.
  • Adequately documents medication orders/changes and observes residents self-administering medications to assure client safety.
  • Attend scheduled staff meetings (program issues, clinical staffing, and ZVMHC all staff) and in-service trainings.
  • Advise administration in a timely manner of requests for vacation, personal leave or conflicts in scheduling. When possible, foreseeable requests for PTO should be made 6 weeks in advance.
  • Maintain data privacy of client records and information. Adhere to all Data Privacy, HIPAA, Vulnerable Adult Regulations, and Zumbro Valley Policies and Procedures.
  • All staff are responsible to help keep the location clean and tidy.
  • Be able to work flexible shifts.
  • Assess the strengths, needs and vulnerability of IRTS residents upon admission. Assess residents for abuse or neglect by self or others on an ongoing basis.
  • Understand and implement emergency procedures for fire, tornado, and winter storms, medical and behavioral emergencies.
  • Maintain contact with other ZVMHC staff, OCDSS and other county case managers, and other community mental health care, primary health care and/or rehabilitation providers regarding IRTS residents.
  • Educate IRTS/CS residents about mental health issues, their medications, mental health symptoms and coping skills.

Qualifications


Requirements and Qualifications

  • Requires an Associates degree in psychology, social work, counseling or related field and 1 year (2000 hours) experience working with adults with a severe mental illness and/or chemical dependency.
  • Must understand the characteristics and needs of adults with a severe mental illness and dually diagnosed.
  • Must be able to work a flexible schedule.
  • Must meet continuing education requirements.
  • 24 hours of continuing education training required every year.
  • Must have MHP or MHPr supervision for 6 hrs/40 hrs worked for the first 160 hours of employment.
  • After the first 160 hours of employment, there must be 6 hours of supervision for every 6 months. Notes must be co-signed by an MHP or MHPr who directly observes the work of the rehab worker.
