Description

The Case Manager’s responsibility is always to Role Model appropriate behavior while mentoring students in educational, recreational, leisure and vocational settings. The Case Manager’s primary responsibility is the overall development and implementation of the youth’s service and intervention/treatment plan. 


Able to obtain Certification in Physical Intervention, 

Able to obtain Certification in CPR, First Aid by a nationally recognized organization (Red Cross, AHA). 

Current state driver’s license (CDL may be required). 

Bachelor’s degree in the behavioral sciences, human services, or social services fields. Case management experience is strongly encouraged.   

Meet criminal background check and drug screening per state requirements. 

Requirements

  •  Provide care to meet the physical, emotional, and social needs of the youth in the residential commitment setting. 
  • Conduct intake and program orientation procedures with each new youth in accordance with program policy. 
  • Provide each youth, parent, guardian, juvenile probation officer, youth attorney or guardian ad litem and the Department a copy of the written policies that encourage and support family visits, mail, telephone calls, and other forms of communication with parents, relatives, friends, or others with whom the child may have a significant relationship. 
  • Provide a written agreement to outline frequency of contact with the child’s family and staff from the agency, a plan for sharing information about the child’s care and development, visitation plans, ongoing evaluation of needs and progress, conditions for discharge, and post-release services. 
  • Provide a trauma informed approach and consider each child’s needs and trauma history when making room assignments. 
  • Develop a written service plan or treatment plan in accordance with all program policy and procedures.  Conduct all training as required by the contracted agency. 
  • Prepare a written discharge summary and discharge planning according to policy. 
  • Organizes, develops, and ensures the efficient transition of all youth entering the program and then exiting back into their home community. Organizes, conducts, and coordinates all transitional planning and services for each youth. 
  • Participates in the development and/or revisions of transition goals on the service/treatment plan. 
  • Effectively implement the behavior motivation system. 
  • Effectively address youth’s unacceptable behavior through verbal dialog and documentation. 
  • Interact with students appropriately while in vocational, educational, work, and leisure settings. 
  • Prevent and intervene in youth altercations and incidents. 
  • Communicate all incidents regarding youth to the appropriate DJJ personnel. 
  • Effectively communicate student information and concerns with other employees. 
  • Maintain all youth information confidentiality and update the Juvenile Justice Information System (JJIS) as required by policy and administrative rule 63E-7. 
  • Complete all documentation in a timely manner and according to program policy and procedure. 
  • Drive Twin Oaks vehicles as required in accordance with transportation policy. 
  • If needed, will provide emergency First Aid and CPR in accordance with American Red Cross Standards. 
  • Participate in emergency evacuation procedures. 
  • Attend and participate in all required meetings and trainings. 
  • Assist each youth in meeting their goals while in the program. 
  • Adhere to all program policies and procedures.
  • Assume other duties, responsibilities, shifts and/or hours as directed or needed for efficient operations of the program.
