On Call Registered Nurse -- Hospice

CareFor is looking for a skilled, compassionate RN to support patients when it matters most.
We’re seeking a dependable and responsive On-Call Registered Nurse to join our growing team. This role is ideal for someone who thrives in high-touch, high-impact moments and is confident making clinical decisions independently in the field.
This position requires local travel throughout the Austin metro area (Georgetown to San Marcos). The On-Call RN must reside within this region and be available to respond in person during all on-call hours to support Hospice clients.
🕑 Schedule & Compensation:

Full time phone triage during all on-call hours every week
-
7 days on / 7 days off schedule for active clinical work during on-call hours

On-call hours:
-
Weeknights: 5:00 PM – 8:00 AM

Weekends: 24-hour availability

Daily compensation during all on-call days

Additional hourly compensation for active clinical work performed
🩺 What You'll Do:
-
Respond quickly and compassionately to after-hours needs from CareFor’s hospice clients.

Provide phone triage and determine if in-home visits are necessary.

Perform in-person assessments to manage new symptoms, urgent needs, or changes in condition.

Administer medications and treatments per plan of care or physician orders to relieve pain and manage distressing symptoms.

Offer crisis intervention and symptom management in coordination with the broader care team.

Communicate clearly and empathetically with families, caregivers, physicians, and interdisciplinary colleagues to ensure seamless transitions and continuity of care.

Maintain timely and thorough documentation for every patient interaction.

Have professional hospice nursing experience and feel confident managing urgent or end-of-life scenarios independently.

Can perform a wide range of clinical tasks including wound care, G-tube feedings, trach care, and more.

Are comfortable navigating diverse home environments and medical situations.

✅ Required Qualifications:

Active Registered Nurse (RN) license in the state of Texas

Residency within the Austin area (Georgetown to San Marcos)

Reliable transportation and readiness to travel locally at a moment’s notice

Availability to work 7 days on / 7 days off, including evenings, overnights, and weekends
