Job Title: Care Team Supervisor

Elara Caring is seeking a dedicated and visionary Care Team Supervisor to join our leadership team in Nacogdoches, TX. This role is ideal for a leader who is passionate about patient care and committed to improving clinical outcomes through operational excellence and quality assurance programs. As a Care Team Supervisor, you will play a crucial role in shaping the future of healthcare by developing and guiding a compassionate, high-performing team of healthcare providers.

Key Responsibilities:

  • Lead, mentor, and support a team of healthcare providers to ensure high-quality patient care and clinical outcomes.

  • Implement and oversee Quality Assurance and Performance Improvement (QAPI) programs.

  • Manage day-to-day operational functions, ensuring compliance with company policies and regulatory requirements.

  • Collaborate with clinical and administrative teams to enhance service delivery and patient satisfaction.

  • Monitor team performance and provide regular feedback and coaching.

  • Ensure adherence to Medicare/Medicaid guidelines, home care benefits, and state-specific policies.

  • Promote a positive, patient-centered work environment.

Qualifications:

  • High School Diploma or GED required.

  • Minimum of 2 years experience in a fast-paced office or healthcare environment preferred.

  • Knowledge of Medicare/Medicaid, home care benefits, and applicable state policies preferred.

  • Strong computer proficiency and excellent communication skills.

  • Ability to thrive in a fast-paced, dynamic work environment.

  • Genuine passion for patient care and team development.

  • Reliable transportation to meet job responsibilities.
