About the Department
Hennepin County's Human Services and Public Health department is seeking a Forensic Case Manager to work in the Adult Behavioral Health Case Management area on the Forensic Case Management team. In this position, you will provide specialized mental health targeted case management services to adults with complex psychiatric and behavioral disorders who are indeterminately committed as mentally ill and dangerous to the public. The Forensic Case Management team monitors the progress of residents admitted to the Forensic Mental Health program and are an active participant in supporting residents as they transition to the community. Forensic Case Managers provide intensive case management services to residents who reside in various community settings. They collaborate with both formal and informal support systems to ensure that each resident's needs are effectively met. Regardless of where the resident resides, we work closely with the person, their supports, their treatment team, and other team members to support the highest level of community integration attainable.

Location and hours:
This position is hybrid and will be performed at various community locations including congregate living facilities throughout the state of Minnesota, resident's homes, the Hennepin County Public Safety Facility at 401 4th Avenue South, Minneapolis, MN 55415, the Forensic Mental Health Hospital at 100 Freeman Drive, St. Peter, MN 56082, and remote as job duties require. Work hours will be Monday through Friday, between 8 a.m. - 4:30 p.m. No weekend, overnight, or holiday shifts required. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin. New Employee Orientation (NEO) for all new benefit-earning employees is in-person in downtown Minneapolis. Details about NEO will be provided upon hire.

In this position, you will:
- Provide and document adult targeted case management (including assessment, planning, referral linkage, follow up, and crisis intervention) to people indeterminately civilly committed as mentally ill and dangerous to the public.
- Develop, implement, and monitor provisional discharge plans for residents in community settings as the designated agency, in collaboration with the treatment facility or state-operated treatment program to which the person is committed.
- Meet with clients in a variety of community settings, including their homes, the community, in public safety facilities, and in state-operated treatment programs, to establish rapport, engage, and perform culturally responsive clinical assessments of strengths, needs, values, preferences, and available supports.
- Mutually identify goals and strategies, including accessing services such as psychiatric and/or substance use treatment, medication management, housing, vocational development, financial assistance, and other ongoing support needs.
- Work collaboratively with interdisciplinary team members for consultation, coordination, and coverage.
- Refer to and coordinate community services that are in compliance with resident's provisional discharge, that are developed in collaboration with the resident and responsive to the social, cultural, emotional, and care needs specific to the person.
- Focus on psychiatric and/or addiction care, medication management, housing, vocational development, financial assistance, and other ongoing support needs.
- Act as a team lead, as needed, to ensure referrals are initiated and completed with the most appropriate services and supports.
- Partner with the County Attorney’s Office and civil and criminal courts to support the best outcome and community services plan.
Need to have:
- Master's degree or higher in social work or closely related field and two or more years of post-master's degree experience in a psychiatric or mental health setting.
- Qualification as a Mental Health Professional in the state of Minnesota.
- Current licensure, or eligibility for licensure within six months in the state of Minnesota, as one or more of the following:
- Independent Clinical Social Worker (LICSW) by the Minnesota Board of Social Work
- Licensed Marriage & Family Therapist (LMFT)
- Professional Clinical Counselor (LPCC) by the Minnesota Board of Behavioral Health and Therapy
- Licensed Psychologist (LP)
- A valid driver's license and access to reliable transportation to travel to meet with clients in their residential settings and/or perform authorized off-site work duties, and the ability to obtain a Hennepin County driver's permit to provide transportation as needed to essential services.

- Experience:
- Providing case management.
- Serving people with significant mental illness and substance use disorder.
- Conducting clinical assessments, preparing discharge planning, or serving people with civil commitment orders.
- Working with Medical Assistance, disability waivers, and managed care.
- Knowledge of:
- Diversity of cultural and lifestyle experiences, beliefs, and practices relating to spiritual, mental, and physical health, community, family structure, and dynamics and disparities.
- Person-centered practice and how it applies to serving people on an involuntary basis.
- Social work best practices.
- Community resources and providers.
- Ability to communicate fluently in another language in addition to English, both verbally and in writing.
