For individuals transitioning from closed custody correctional facilities, Cherry Health provides Person Centered Reentry Services to ease the transition back into the community. Services are available at two locations: our Community Treatment Center (CTC) in Detroit and Community Alternatives Program (CAP) in Grand Rapids. Services include 24-hour residential treatment in a structured environment with individual and group counseling services. Some participants come to us directly from a correctional facility, while others come as part of a diversion program to keep them out of prison. Together, we work to address health issues (including behavioral health issues) and other reentry needs i.e., employment, education, housing, and fostering connections to help justice system involved persons successfully reintegrate back into their respective communities.
OVERVIEW OF POSITION:
This position is responsible for the direct provision or coordination of assessment and treatment of assigned clients. This includes, but is not limited to intake assessment and brief report for all new admissions, treatment programming, and coordination of residents’ activities. Also conducts at least one pro-social group outside of CTS groups. Must adhere to the organizational Policies, Procedures, Code of Ethics, and all licensing and accreditation standards of the position. Must represent the organization’s Mission in all actions and communications.
This is a full time, benefit eligible position working typically Monday - Friday onsite at our Community Alternatives Program.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Facilitate client intakes.
- Provide resident assessments, treatment plans, individual sessions, and group sessions.
- Case conference presentations.
- Maintain timely clinical documentation and reporting within all standards and agency policies which apply.
- Communicate thoroughly with the clinical team.
- Become familiar with and maintain standards of Federal Bureau of Prisons guidelines as outlined in the FBOP Statement of Works.
- Maintain a caseload of up to 24 residents, including CTS after-care programming.
- Understand and adhere to compliance standards as they appear in the Corporate Compliance Policy, Code of Conduct, and Conflict of Interest Policy.
- Keep abreast of all pertinent federal, state, and CSHS’s regulations, laws, and policies as they presently exist and as they change or are modified.
- Other duties as assigned.
SKILLS / KNOWLEDGE / ABILITIES:
- Must be able to demonstrate knowledge of criminogenic factors, current addiction theories, and a variety of cognitive behavioral therapies.

EDUCATION / LICENSES / CERTIFICATES:
- Master’s Degree in Social Work or Licensed Professional Counseling required. LLMSW, LMSW, LLPC, LPC.
- Must have a CAADC certification through Michigan Certification Board for Addiction Professionals (MCBAP) or be eligible to file a development plan to complete credentialing.
EXPERIENCE:
Three to five years’ experience in case management, biopsychosocial assessments, treatment plans, Crisis Intervention, or CTS programming; or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job listed above.
PHYSICAL REQUIREMENTS:
Possible exposure to patients/clients with illnesses and/or infections. Frequent telephone and computer use, requiring manual dexterity for writing and computer use. Sitting, standing or walking for extended periods of time. Specific vision abilities required by this job may include close vision, distance vision, and ability to adjust focus. May require bending, squatting, or kneeling and lifting up to 50 pounds. Use of Personal Protective Equipment (PPE) as required.
