JOB DESCRIPTION SUMMARY
The position will be responsible for oversight and coordination of patient care for assigned hospice patients and for providing skilled palliative care to hospice patients. Applicants should have excellent observation, clinical, and interpersonal skills, and have good nursing judgment. Position will work closely with the established home health team to ensure appropriate patient care and outcomes. Applicant must have the ability to communicate and interact with patients and family members, and should be able to work independently and organize/prioritize a schedule.
Position Summary Expanded:
Under the Oklahoma Medical Marijuana and Patient Protection Act, (“The Unity Bill”) this position is considered a “Safety-sensitive position. “Safety-sensitive” is defined to include “any job that includes tasks or duties that could affect the safety and health of the employee performing the task or others.” Employees working in “safety-sensitive” roles are subject to this exception and subject to disciplinary action in the event of a positive test for marijuana or its metabolites.

Physical Requirements
- Light to moderate physical activity required, including possible lifting to 50lbs, and turning of adult clients.
- Good color vision is essential to determine subtle changes in a patient’s skin color and to rapidly recognize medication and color-coded supplies.
- Requires good hearing to assess situations quickly and respond appropriately.
Position Qualifications:
- Currently licensed to practice as a Registered Nurse in the State of Oklahoma.
- At least 2 years of Home Health experience as an RN preferred.
- Proficient in verbal, written, and electronic communication.
- Must be flexible to adjust to changing tasks frequently.
- Must have a valid driver’s license, reliable vehicle, maintain valid auto liability insurance, and be able to drive throughout the service area under various weather conditions.

Education/Experience
- BSN and/or MSN
- Three to five years related experience and/or training; or equivalent combination of education and experience.
Certificates and Licenses
- Valid Driver’s License
- Current auto insurance verification
- CPR Certification
- Currently licensed to practice as a Registered Nurse in the State of Oklahoma

LANGUAGE SKILLS
English is the primary language of the facility. Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to effectively present information and respond to questions from groups of executives, managers, clients, customers, and the public.
MATHEMATICAL SKILLS
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions and decimals.
REASONING ABILITY
Ability to apply common sense understanding to carry out instructions furnished in written, oral, and/or diagram form. Ability to define and solve problems, interpret data, establish facts, and draw valid conclusions.
COMPUTER SKILLS
To perform this job successfully, an individual should have knowledge of word processing software and spreadsheet software as well as Point of Care systems. Must also have experience with internet and email systems.
