Description
- Complete biopsychosocial assessments within 72 hours of admission and revise treatment plans weekly to reflect progress and evolving needs.
- Facilitate two clinical group therapy sessions per day, using an evidence-based curriculum (e.g., relapse prevention, early recovery, CBT).
- Provide weekly individual and/or family counseling sessions, tailored to the individual's stage of recovery.
- Actively participate in daily clinical meetings and weekly treatment team reviews to assess progress and adjust interventions.
- Document assessments, treatment plans, progress notes, and discharge summaries in a timely and accurate manner in the Electronic Health Record (EHR).
- Initiate and coordinate discharge planning upon admission, ensuring all aftercare appointments and community referrals are secured.
- Assess the need for level-of-care transitions and make appropriate referrals, including from detox to rehab or to lower-intensity services.
- Collaborate with nursing and medical
- Provide support during crises using trauma-informed, de-escalation techniques (e.g., CPI).
- Orient new participants to the rehab program and help set expectations around participation and group engagement.
- Maintain a therapeutic and professional environment by modeling clear boundaries, respect, and recovery-focused communication.
- Ensure detail-oriented clinical documentation complies with licensing, payors, and accreditation standards (e.g., ASAM, Joint Commission).
- Maintain clinical records that support diagnoses, treatment goals, progress, and discharge readiness.
- Perform other duties as assigned
Qualifications
Education:
- Required: Master’s degree in a behavioral health related field. CPR and hospital-selected de-escalation technique certification. Current CAADC or acquire certification within six (6) months of hire date.
- Preferred: Clinical license preferred.
- Maintains education and development appropriate for position.
Experience
- Required: One (1) year experience with substance use in an inpatient or rehab setting
- Preferred: Previous assessment and evaluation experience
