Position: Case Manager 1
Program: Women’s Community Transition Support Program
Location: Middletown
Salary: $20.00-21.00/hour
Schedule: FT/40 hours/1st shift/Monday-Friday
Position Summary:
Are you interested in playing a pivotal role in the lives of families and children? If you are, come join the exciting Women’s Community Transition Program team as a Admin Specialist! In this role, you will support clients in achieving their individual goals by providing intensive in-home case management services. You will help families address the problems of homelessness, unemployment, mental illness, and substance use through collaboration with treatment providers, natural supports, and community resources.
Working within our Family & Child Services, the Case Manager 1 works with the agency staff to provide services to stabilize and reunite families experiencing complex issues such as substance use, behavioral health matters, and homelessness.
Requirements:
- Some post-secondary education
- 2+ years related experience in the direct service and case management field
- Valid Connecticut Driver’s License
- Regular and predictable attendance is required
Orientation:
If selected for employment with The Connection, newly hired employees will be required to attend a two (2) day mandatory New Hire Orientation at our Roscommon office located in Middletown.
