Qualifications: 

  • High school diploma or certificate of successful completion of the General Educational Development Test.
  • Have three (3) years of responsible full time clerical experience

Duties and Responsibilities: 

  1. Demonstrate ability to engage individuals served in a welcoming, hopeful, empathetic manner regardless of disability or phase of recovery.
  2. Collaborate with primary care physicians, mental health specialists, and other healthcare professionals to ensure that individuals receive comprehensive integrated care for both physical and mental health.
  3. Monitor initial and ongoing health screenings, including but not limited to medical history, behavioral health issues, and any co-occurring disorders.
  4. Work with multidisciplinary teams to support the development of personalized, holistic person-centered treatment plans that address both mental health disorders (e.g., depression, anxiety, substance use) and physical health conditions (e.g., diabetes, hypertension).
  5. Gather materials and/or resources to share with care teams.
  6. Monitor individuals progress and facilitating regular follow-up appointments to ensure continuity of care. This includes providing referrals and resources to specialty services and/or MyCare, when needed.
  7. Understand the link between mental and physical health and encourage healthy lifestyle choices that support the overall well-being of the person served.
